健脾益肾活血法对症治疗慢性肾脏病3~5期疗效观察

Clinical observation on invigorating the spleen,nourishing the kidney and activating blood circulation method on chronic kidney disease stage 3~5

目的观察自拟健脾益肾活血中药汤剂联合西医对症治疗应用于慢性肾脏病3~5期的临床疗效。方法将我院90例慢性肾脏病3~5期患者随机分为治疗组与对照组各45例。对照组给予西医对症支持治疗,治疗组在对照组治疗基础上给予自拟健脾益肾中药汤剂。治疗8周后,比较两组患者的临床疗效,以及治疗前后血肌酐、尿素氮、血磷、血钙水平及中医证候评分。结果治疗后,治疗组总有效率显著高于对照组(P<0.05);两组血肌酐、尿素氮、钙磷水平及中医证候评分均有所改善(P<0.05),组间比较,治疗组在改善血肌酐、尿素氮、钙磷水平及中医症状评分方面均优于对照组(P<0.05)。结论健脾益肾活血中药联合西医对症治疗可以有效改善慢性肾脏病3~5期非透析患者的临床症状,延缓肾脏病进展,促进钙磷代谢平衡,提高临床疗效。

Objective To observe the clinical effect of self-made Chinese herbal decoction for invigorating the spleen and nourishing the kidney combined with western medicine for symptomatic treatment of chronic kidney disease stage 3~5.Methods Ninety patients with chronic kidney disease stage 3~5 were selected and randomly divided into treatment group and control group with 45 cases in each group.Control group was given western medicine symptomatic and supportive treatment,and treatment group was given self-made Chinese herbal decoction for invigorating the spleen and nourishing the kidney on the basis of control group.After 8 weeks of treatment,the clinical efficacy,renal function,serum phosphorus,serum calcium levels and TCM syndrome scores before and after treatment were compared between the two groups.Results After treatment,the total effective rate of treatment group was significantly higher than that of control group(P<0.05);blood creatinine,urea nitrogen,calcium and phosphorus levels,and TCM symptom scores were improved in both groups(P<0.05),and were high in treatment group than in control group(P<0.05).Conclusion Chinese medicine for invigorating spleen,nourishing kidney and activating blood circulation combined with symptomatic treatment of western medicine could effectively improve the clinical symptoms of non-dialysis patients with chronic kidney disease stage 3~5,delay the progression of kidney disease,promote the balance of calcium and phosphorus metabolism,and improve clinical efficacy.
