针灸联合西药治疗肥胖多囊卵巢综合征患者疗效观察

Clinical observation on therapeutic effect of acupuncture combined with Western medicine on obese patients with polycystic ovary syndrome

目的观察针灸联合枸橼酸氯米芬(CC)、尿促性腺激素(HMG)、绒毛膜促性腺激素(HCG)治疗肥胖多囊卵巢综合征(PCOS)临床疗效。方法将我院120例肥胖PCOS患者随机分为对照组与针灸组各60例。对照组予以调节饮食、CC、HMG、HCG治疗,观察组在对照组治疗基础上予以针灸治疗。比较两组临床疗效,体质量指数(BMI)、体质量(BW)、腰臀比(WHR),促黄体生成素(LH)、睾酮(T)、胰岛素抵抗指数(HOMA-IR)水平,卵巢体积、子宫内膜厚度、月经周期。结果治疗后,针灸组总有效率高于对照组(P<0.05);两组BMI、BW、WHR值均较治疗前下降,针灸组BMI、BW、WHR值均低于对照组(P<0.05);两组LH、T、HOMA-IR水平均较治疗前下降,针灸组LH、T、HOMA-IR水平均低于对照组(P<0.05);两组卵巢体积、月经周期均较治疗前减小而子宫内膜厚度均较治疗前增加,针灸组卵巢体积、月经周期均小于对照组而子宫内膜厚度均多于对照组(P<0.05)。结论针灸联合CC、HMG、HCG治疗肥胖PCOS患者疗效显著,能显著降低体质量与卵巢体积,促进月经周期恢复正常,调节性激素水平,改善胰岛素抵抗。

Objective To observe the therapeutic effect of acupuncture combined with clomiphene citrate(CC)/urinary gonadotropin(HMG)/chorionic gonadotropin(HCG)on obese patients with polycystic ovary syndrome(PCOS).Methods A total of 120 obese patients with PCOS were randomly divided into control group and acupuncture-moxibustion group with 60cases in each group.Control group was treated with adjusted diet and CC/HMG/HCG,and acupuncture group received acupuncture treatment on the basis of control group.The clinical efficacy,body mass index(BMI),body mass(BW),waist-hip ratio(WHR),luteinizing hormone(LH),testosterone(T),insulin resistance index(HOMA-IR)levels,ovarian volume,endometrial thickness,menstrual cycle were compared.Results After treatment,the total effective rate of tacupuncture group was higher than that of control group(P<0.05);the BMI,BW and WHR values were decreased compared with those before treatment in both groups,and were lower in acupuncture group than in control group(P<0.05);the levels of LH,T,and HOMA-IR were decreased compared with those before treatment in both groups,and were lower in acupuncture group than in control group(P<0.05);the ovarian volume and menstrual cycle decreased,and the endometrial thickness increased compared with those before treatment in both groups,acupuncture group had smaller ovarian volume and menstrual cycle,and larger endometrial thickness(P<0.05).Conclusion Acupuncture combined with CC/HMG/HCG has a significant effect in treatment of obese PCOS patients by reducing body weight and ovarian volume,promoting the normalization of menstrual cycle,regulating sex hormone levels,and improving insulin resistance.
