益气祛风活络汤联合针灸治疗顽固性面瘫疗效观察

Effects of Yiqi Qufeng Huoluo Decoction combined with acupuncture on symptom scores and electromyography in patients with intractable facial paralysis

目的观察益气祛风活络汤联合针灸治疗顽固性面瘫临床疗效。方法将我院收治50例顽固性面瘫患者随机分为对照组与研究组各25例。对照组采用西药治疗,研究组在对照组治疗基础上采用益气祛风活络汤联合针灸治疗。观察两组临床疗效,治疗前后面神经麻痹症状、面部残疾程度(面部残疾指数躯体功能指数FDIP、社会生活功能FDIS)、面神经功能评分系统House-Brackmann(H-B)、肌电图(眼、口轮匝肌振幅)。结果治疗后,研究组总有效率高于对照组(P<0.05);两组抬额、皱眉、闭眼、耸鼻、鼓腮、示齿、患侧面肌障碍评分均明显高于治疗前(P<0.05),研究组上述指标评分高于对照组(P<0.05);两组FDIP分值明显高于治疗前,FDIS、H-B分值均明显低于治疗前(P<0.05),研究组FDIP分值高于对照组,FDIS、HB分值低于对照组(P<0.05);两组眼、口轮匝肌振幅均明显高于治疗前(P<0.05),研究组眼、口轮匝肌振幅高于对照组(P<0.05)。结论益气祛风活络汤联合针灸更能减轻顽固性面瘫患者面神经麻痹症状,缓解面部残疾程度,提高面神经功能,改善面部肌电活动。

Objective To observe the effect of Yiqi Qufeng Huoluo Decoction combined with acupuncture on symptom scores and electromyography in patients with intractable facial paralysis.Methods Fifty patients with intractable facial paralysis were randomly divided into control group and research group,25 cases in each group.Control group was treated with western medicine,and study group was treated with Yiqi Qufeng Huoluo Decoction combined with acupuncture and moxibustion on the basis of control group.The curative effects after treatment,the symptoms of facial nerve paralysis,facial disability index(physical function index,FDIP;social life function,FDIS),facial nerve function scoring system House-Brackmann(H-B),electromyography(eye,orbicularis oris muscle amplitude)of the two groups were observed.Results After treatment,the total effective rate of study group was higher than that of control group(P<0.05);the scores of forehead lift,frowning,eyes closed,nose cocking,bulging cheeks,showing teeth,muscle disorder on the affected side were significantly higher than those before treatment in both groups(P<0.05);forehead lift,frowning,eyes closed,nose cocking,bulging cheeks,showing teeth,muscle disorder on the affected side socres were higher in study group than control group(P<0.05);the FDIP scores were significantly higher than those before treatment,and were higher in study group;the scores of FDIS and H-B were significantly lower than those before treatment in both groups(P<0.05),and were lower in study group than control group(P<0.05);the amplitudes of eye and orbicularis oris muscle were significantly higher than those before treatment in both groups(P<0.05),and were higher in study group than control group(P<0.05).Conclusion Yiqi Qufeng Huoluo Decoction combined with acupuncture can better relieve the symptoms of facial paralysis in patients with intractable facial paralysis,alleviate the degree of facial disability,improve facial nerve function,and improve facial EMG activity.
