针刺联合补肾解郁方治疗多囊卵巢综合征性不孕症(肝郁肾虚型)临床疗效及对卵巢功能的影响

Clinical curative effect of acupuncture combined with Bushen Jieyu Decoction on infertility of polycystic ovary syndrome of liver-stagnation kidney deficiency type and its influences on ovarian function

目的观察针刺联合补肾解郁方治疗多囊卵巢综合征(PCOS)性不孕症(肝郁肾虚型)临床疗效及对卵巢功能的影响。方法将我院80例肝郁肾虚型PCOS不孕症患者随机分为观察组与对照组各40例。对照组采用针刺治疗,观察组采用针刺联合补肾解郁方治疗,共治疗3个月经周期。分别于治疗前后记录两组患者治疗总有效率和中医证候积分,采用化学发光法检测血清促卵泡生成素(FSH)、促黄体生成素(LH)、睾酮(T)、雌二醇(E2)和泌乳素(PRL)水平。采用阴道彩色多普勒超声检测卵巢功能。治疗后观察排卵和妊娠情况。结果治疗后,观察组总有效率显著高于对照组(P<0.05);观察组中医证候积分明显低于对照组(P<0.05);两组治疗后血清激素水平均有所改善,观察组患者血清LH、FSH、T、PRL水平明显低于对照组,E2水平明显高于对照组(P<0.05);观察组卵泡减少数量、最大卵泡直径和子宫内膜厚度均明显大于对照组(P<0.05);观察组排卵率和妊娠率明显高于对照组(P<0.05)。结论针刺联合补肾解郁方治疗PCOS不孕症的临床疗效好,能够改善患者激素水平,恢复卵巢功能,提高排卵率和妊娠率,值得临床推广应用。

Objective To observe the clinical curative effect of acupuncture combined with infertility of Bushen Jieyu Decoction on polycystic ovary syndrome(PCOS)of liver-stagnation kidney deficiency type and its influences on ovarian function.Methods Eighty patients with PCOS induced infertility of liver-stagnation kidney deficiency type were randomly divided into observation group and control group,40 cases in each group.Control group was treated with acupuncture,while observation group was additionally treated with Bushen Jieyu Decoction.Both groups were treated for 3 menstrual cycles.The total response rates of treatment and scores of TCM syndromes in both groups before and after treatment were recorded,the levels of serum follicle-stimulating hormone(FSH),luteinizing hormone(LH),testosterone(T),estradiol(E2)and prolactin(PRL)were detected by chemiluminescence,the ovarian function was tested by transvaginal color Doppler ultrasonography,the ovulation and pregnancy were observed after treatment.Results After treatment,total response rate in observation group was significantly higher than that in control group(P<0.05),and scores of TCM syndromes were significantly lower than those in control group(P<0.05);the levels of serum hormones were improved in both groups,the levels of serum LH,FSH,T and PRL in observation group were significantly lower than those in control group,and E2level was significantly higher than that in control group(P<0.05);the number of reduced follicles,the maximum diameter of follicle and endometrial thickness in observation group were significantly higher than those in control group(P<0.05);the ovulation rate and pregnancy rate were significantly higher than those in control group(P<0.05).Conclusion Acupuncture combined with Bushen Jieyu Decoction is effective on PCOS infertility by improving hormones,recovering ovarian function,increasing ovulation rate and pregnancy rate.
