早产儿中医体质类型分布及中西医病因病机分析

Distribution of TCM constitution types of premature infants and analysis of etiology and pathogenesis of Chinese and western medicine

目的分析早产儿中医体质类型分布情况,并对其中西医病因病机进行研究与总结。方法选取2019年1月—2022年1月清远地区120例早产儿及其母亲作为研究对象,另选取同期100例健康足月儿及其母亲作为健康对照组。基于中华中医药学会《中医体质分类与判定》标准对新生儿及其母亲中医体质进行调查与评估,观察早产儿与足月健康儿及其母亲中医体质分布情况,并调查早产的发生原因。结果在中医体质类型分布调查中发现,足月儿中医体质主要以平和质为主,而早产儿中医体质则以偏颇质为主,二者中医体质分布比较,差异有统计学意义(P<0.05)。足月儿母亲中医体质主要以平和质为主,早产儿母亲中医体质主要以偏颇质为主,其中以阳虚质、气虚质最为常见,二者中医体质分布比较,差异有统计学意义(P<0.05)。在对120例早产儿发生原因调查中发现,胎膜早破、妊高症、胎盘脐带等因素是导致早产发生的主要原因。结论早产儿及其母亲中医体质以偏颇体质最为常见,且早产儿的发生与母亲中医体质偏颇具有一定相关性,因此在女性孕前保健以及孕中检查是了解其中医体质类型,并及早干预其偏颇体质,对预防早产以及提高新生儿质量具有重要的临床意义。

Objective To analyze the distribution of TCM constitution types of preterm infants,and study and summarize the etiology and pathogenesis of Chinese and western medicine.Methods A total of 120 premature infants and their mothers in Qingyuan from January 2019 to January 2022 were selected as the research object,and 100 healthy full-term infants and their mothers in the same period were selected as healthy control group.Based on the standard of classification and judgment of Traditional Chinese Medicine Constitution issued by Chinese Society of Traditional Chinese Medicine,the TCM constitution of newborns and their mothers was investigated and evaluated,the distribution of TCM constitution types of premature infants and their correlation with their mothers’TCM constitution were observed.Observe the distribution of TCM constitution of preterm infants,full-term healthy infants and their mothers,and investigate the causes of preterm birth.Results It was found that the TCM constitution of term infants was mainly peaceful,while that of preterm infants was mainly biased,there was significant difference in the distribution of TCM constitution(P<0.05);the TCM constitution of term infants’mothers was mainly peaceful,while that of preterm infants’mothers was mainly biased,among which yang deficiency and qi deficiency were the most common,there was a statistically significant difference in the distribution of TCM constitution(P<0.05);it was found that premature rupture of membranes,pregnancy induced hypertension,placental umbilical cord and other factors were the main causes of premature birth.
