宣肺平喘止咳汤辅助治疗小儿咳嗽变异性哮喘风寒犯肺证的疗效及对肺功能的影响

Efficacy of Xuanfei Pingchuan Zhike Tang (宣肺平喘止咳汤)in adjuvant treatment of pediatric cough variant asthma with wind?cold invading lung syndrome and its effect on lung function

目的观察宣肺平喘止咳汤辅助治疗小儿咳嗽变异性哮喘(CVA)风寒犯肺证的疗效及对肺功能的影响。方法选取2019年1月至2020年12月温州市中西医结合医院儿科收治的CVA风寒犯肺证患儿96例,按照随机数字表法分为对照组和治疗组,各48例。对照组给予西医常规对症治疗,治疗组在对照组治疗方法的基础上予宣肺平喘止咳汤加减治疗。2组均以1周为1个疗程,连续治疗2个疗程后统计疗效。结果对照组总有效率为77.08%(37/48),治疗组为91.67%(44/48),2组比较,差异有统计学意义(P<0.05)。治疗前2组咳嗽、咳痰、咽痒积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组上述积分均显著降低,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组降低更显著(P<0.05)。治疗前2组第1秒用力呼气容积(FEV1)和肺活量(FVC)水平比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组FEV1、FVC水平均明显升高,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组升高更显著(P<0.05)。治疗前2组白细胞介素-17(IL?17)、白细胞介素-23(IL?23)、超敏C-反应蛋白(hs?CRP)水平比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组上述指标水平均明显降低,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组降低更显著(P<0.05)。结论宣肺平喘止咳汤辅助治疗小儿CVA风寒犯肺证疗效显著,可有效缓解临床症状,改善肺功能,减轻炎症反应,值得临床推广应用。

Objective To observe the curative effect of Xuanfei Pingchuan Zhike Tang(宣肺平喘止咳汤)in the treatment of children with cough variant asthma(CVA)of wind-cold invading lung syndrome and its effect on lung function.Methods A total of 96 children with CVA of wind-cold invading lung syndrome who were admitted to Department of Pediatrics,Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine between January 2019 and December 2020 were chosen and divided into control group and treatment group using random number table,with 48 cases in each group.The control group was given conventional symptomatic treatment of Western medicine,and the treatment group was treated with Xuanfei Pingchuan Zhike Tang on the basis of the control group.The two groups were both given 1 week as 1 course of treatment,and we kept the statistics about the efficacy of the two groups before and after 2 courses of continuous treatment.Results The total effective rate was 77.08%(37/48)in the control group and 91.67%(44/48)in the treatment group,the difference being significant(P<0.05).There was no significant difference in the scores of cough,expectoration and pharyngeal itching between the two groups before treatment(P>0.05),indicating that they were comparable.After treatment,the above scores in the two groups were significantly decreased,and the difference was statistically significant compared with the same group before treatment(P<0.05),and the decrease in the treatment group was more significant(P<0.05).There was no significant difference in the levels of forced expiratory volume in one second(FEV1)and vital capacity(FVC)between the two groups before treatment(P>0.05),which was comparable.After treatment the levels of FEV1 and FVC in the two groups were increased significantly when compared with those before treatment in the same group(P<0.05),and the increase in the treatment group was more significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of the levels of interleukin-17(IL-17),interleukin-23(IL-23)and high-sensitivity C-reactive protein(hs-CRP)between the two groups(P>0.05).After treatment,the above indexes in the two groups were decreased significantly,and the difference was statistically significant compared with the same group before treatment(P<0.05),and the decrease in the treatment group was more significant(P<0.05).Conclusion Xuanfei Pingchuan Zhike Tang is effective in the adjuvant treatment of CVA in children with wind-cold invading lung syndrome.It can relieve clinical symptoms effectively,improve lung function and reduce inflammatory response,which is worthy of clinical application.
