穴位埋线治疗腰椎间盘突出症的选穴规律探讨

Discussion on Acupoint Selection Rules of Acupoint Catgut Embedding in the Treatment of Lumbar Disc Herniation

目的通过数据挖掘探讨穴位埋线治疗腰椎间盘突出症(LDH)的选穴规律。方法通过中国知网、CBM、维普网、万方数据库、PubMed建库至今收录的穴位埋线治疗LDH的临床文献,建立数据库,对其进行描述性分析、关联规则分析和聚类分析。结果纳入80篇文献,入选135个腧穴处方,累计使用776次,频率较高的穴位有腰夹脊、环跳等19个腧穴,主要归属于膀胱经和胆经,部位以下肢部和腰背部为主;关联规则分析主要为环跳-阳陵泉、环跳-腰夹脊、阳陵泉-腰夹脊、委中-腰夹脊;聚类分析主要类群为环跳、阳陵泉、风市、悬钟、足三里;委中、昆仑、秩边、承山、殷门、承扶;大肠俞、气海俞、关元俞、肾俞;腰阳关、阴陵泉;腰夹脊。结论穴位埋线治疗LDH取穴集中在下肢部和腰背部,以膀胱经和胆经为主,重视腰夹脊和阿是穴等局部腧穴,处方重视辨经选穴。本研究对临床穴位埋线治疗LDH具有参考价值。

Objective To discuss the acupoint selection rules of acupoint catgut embedding in the treatment of lumbar disc herniation(LDH)by means of data mining.Methods Through CNKI,CBM,VIP,Wanfang database,and Pubmed,the clinical literature on the treatment of LDH at acupoints has been established a database for descriptive analysis,association rule analysis and cluster analysis.Results 80 articles were included in the study,and 135 acupoint prescriptions were selected,which used it for a total of 776 times.The most frequently used acupoints include 19 acupoints such as Jiaji(EX-B2)and Huantiao(GB30),which are mainly attributed to the bladder meridian and gall bladder meridian.The used points were mostly distributed on lower limbs and lower back.The analysis of association rules is mainly for GB30-Yanglingquan(GB34),GB30-EX-B2,GB34-EX-B2,Weizhong(BL40)-EX-B2.The main groups of cluster analysis are GB30,GB34,Fengshi(GB31),Xuanzhong(GB39),Zusanli(ST36);BL40,Kunlun(BL60),Zhibian(BL54),Chengshan(BL57),Yinmen(BL37),Chengfu(BL36);Dachangshu(BL25),Qihaishu(BL24),Guanyuanyu(BL26),Shenshu(BL23);Yaoyangguan(DU3),Yinlingquan(SP9);EX-B2.Conclusion The acupoint catgut embedding in LDH focuses on the lower limbs and lower back,focusing on the bladder meridian and gall bladder meridian,paying attention to local acupoints such as EX-B2 and Ashi acupoints,paying attention to distinguishing meridians and selecting acupoints.This study has reference value for clinical acupoint catgut embedding in treating LDH.
