多维联合快速康复对腰椎间盘全椎板切除术后的康复效果

Effect of multidimensional combined prerehabilitation on rapid rehabilitation after total lumbar disc laminectomy

目的 观察多维联合加速康复对腰椎间盘全椎板切除术后的康复效果。方法 选取西南医科大学附属医院2020年2月至2022年2月收治的86例脊柱外科收治的拟行腰椎间盘全椎板切除术患者作为研究对象,采取随机数字表法将其分为对照组和研究组,各43例。对照组予以术后快速预康复训练,研究组予以多维联合快速康复训练,于入院时(干预前)和术后3个月(干预后)采用Oswestry功能障碍指数(ODI)、Melzack的简式疼痛量表(MPQ)评估腰椎功能与疼痛程度。结果 干预后,两组ODI量表各项评分均高于干预前,且研究组高于对照组,差异有统计学意义(P<0.05);两组MPQ各评分均低于干预前,且研究组低于对照组,差异有统计学意义(P<0.05)。结论 腰椎间盘突出症全椎板切除术后患者在以快速预康复训练为主的基础上,叠加实施多维康复护理,可有效控制术后疼痛和功能障碍程度,护理模式值得临床广泛推广和深入研究。

Objective To observe the effect of multidimensional combined prerehabilitation on rapid rehabilitation after total lumbar disc laminectomy.Methods A total of 86 patients who were admitted to the Department of Orthopedics Spinal Surgery in Affiliated Hospital of Southwest Medical University were selected as the study subjects from February 2020 to February 2022,the patients were divided into control group and study group,with 43 cases in each group.The control group was given rapid rehabilitation care,and the study group was given multidimensional combined rapid rehabilitation training,Oswestry disability index(ODI) and Melzack’s simple pain scale(MPQ) were used to assess lumbar function and pain severity at admission(pre-intervention),and three months after surgery(post-intervention).Results After intervention,ODI scores of both groups were higher than before intervention,and the study group was higher than the control group,the differences were statistically significant(P<0.05).MPQ scores in both groups were lower than before intervention,and the study group was lower than the control group,the differences were statistically significant(P<0.05).Conclusion After total laminectomy for lumbar disc herniation patients,on the basis of rapid pre-rehabilitation training,superimposed multidimensional rehabilitation nursing can effectively control the degree of postoperative pain and dysfunction.The nursing mode is worthy of extensive clinical promotion and in-depth research.
