口腔种植修复术对慢性牙周炎患者牙周指数及MMP-2水平的影响探讨

摘要 目的观察口腔种植修复对慢性牙周炎患者牙周指数及基质金属蛋白酶-2(MMP-2)水平的影响。方法82例行口腔种植修复术的牙列缺损患者,按照是否伴慢性牙周炎分为对照组(牙周健康)与实验组(伴慢性牙周炎),每组41例。对照组患者行常规口腔种植修复术治疗,实验组患者在行口腔种植修复术前需完成系统牙周炎治疗。比较两组患者术后6、12个月牙周指数[牙周探诊深度(PD)、龈沟出血指数(SBI)、菌斑指数(PLI)]以及MMP-2水平、术后12个月种植体存活率。结果术后6个月,实验组患者PD、SBI、PLI、MMP-2水平分别为(2.12±0.08)mm、(1.01±0.04)分、(0.94±0.03)分、(122.11±10.36)ng/L,对照组患者分别为(1.75±0.06)mm、(0.75±0.07)分、(0.91±0.05)分、(95.07±10.35)ng/L;术后12个月,实验组患者PD、SBI、PLI、MMP-2水平分别为(2.71±0.12)mm、(1.52±0.06)分、(1.55±0.07)分、(132.32±10.35)ng/L,对照组患者分别为(2.01±0.11)mm、(1.04±0.04)分、(1.05±0.05)分、(115.42±10.33)ng/L。术后12个月,两组患者PD、SBI、PLI、MMP-2水平均高于本组术后6个月,差异具有统计学意义(P<0.05);术后6、12个月,实验组患者PD、SBI、PLI、MMP-2水平均高于对照组,差异具有统计学意义(P<0.05)。实验组患者术后12个月种植体存活率97.56%低于对照组的100.00%,但差异无统计学意义(P>0.05)。结论牙列缺损伴慢性牙周炎患者口腔种植修复术后牙周指数及MMP-2水平均高于牙周健康者,但是术后12个月随访种植体存活率与牙周健康者无明显差异。 Objective To observe the effect of dental implant restoration on periodontal index and matrix metalloproteinase-2(MMP-2)level in patients with chronic periodontitis.Methods A total of 82 patients with dental implant restoration were divided into a control group(periodontally healthy)and an experimental group(with chronic periodontitis)according to whether they had chronic periodontitis or not,with 41 cases in each group.Patients in the control group were treated with conventional dental implant restoration,while patients in the experimental group were required to complete systematic periodontitis treatment before dental implant restoration.Both groups were compared in terms of periodontal indices[probing depth(PD),sulcus bleeding index(SBI),plaque index(PLI)]and MMP-2 levels at 6 and 12 months after surgery,and the implant survival rate at 12 months after surgery.Results At 6 months after surgery,the PD,SBI,PLI,and MMP-2 level were(2.12±0.08)mm,(1.01±0.04)points,(0.94±0.03)points,and(122.11±10.36)ng/L in the experimental group,and(1.75±0.06)mm,(0.75±0.07)points,(0.91±0.05)points,and(95.07±10.35)ng/L in the control group.At 12 months after surgery,the PD,SBI,PLI,and MMP-2 level were(2.71±0.12)mm,(1.52±0.06)min,(1.55±0.07)min,and(132.32±10.35)ng/L in the experimental group,and(2.01±0.11)mm,(1.04±0.04)min,(1.05±0.05)points,and(115.42±10.33)ng/L in the control group.At 12 months after surgery,the PD,SBI,PLI,and MMP-2 level in both groups were higher than those at 6 months after surgery,and the differences were statistically significant(P<0.05).At 6 and 12 months after surgery,the PD,SBI,PLI,and MMP-2 level in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).At 12 months after surgery,the implant survival rate was 97.56%,which was lower than 100.00%in the control group,but the difference was not statistically significant(P>0.05).Conclusion The periodontal index and MMP-2 level of patients with chronic periodontitis after dental implant restoration are higher than those of healthy periodontal patients,but the implant survival rate at 12-month postoperative follow-up was not significantly different from that of the healthy periodontal patients.
