益生菌防治儿童抗生素相关性腹泻的临床价值观察

摘要 目的分析儿童抗生素相关性腹泻(AAD)疾病特点,研究益生菌防治的临床价值。方法60例行抗生素治疗的下呼吸道感染患儿,依据随机数字表法分为对照组和观察组,每组30例。对照组采取常规干预,观察组采取益生菌防治干预。比较两组患儿抗生素应用不同时间(1、3、6d)AAD发生情况,干预后乳酸杆菌菌群数、腹泻指标积分。结果抗生素应用1、3、6d,观察组患儿AAD发生率分别为3.33%、10.00%、16.67%,均低于对照组的23.33%、33.33%、46.67%,差异具有统计学意义(P<0.05)。干预3、6d,观察组患儿乳酸杆菌菌群数为(7.92±1.05)、(8.03±1.20)logcopies/g,高于对照组的(6.58±0.90)、(6.30±0.90)logcopies/g;大便形状积分为(0.95±0.15)、(0.95±0.15)分,黏液便积分为(0.70±0.05)、(0.48±0.03)分,均低于对照组的(1.25±0.16)、(0.96±0.12)分及(0.96±0.08)、(0.72±0.05)分,差异均具有统计学意义(P<0.05)。结论儿童抗生素应用期间AAD发生情况常见,给予患儿益生菌防治可以有效改善肠道菌群,积极防治AAD,提高儿童应用抗生素期间的安全性。 Objective To analyze the characteristics of antibiotic-associated diarrhea(AAD)in children and study the clinical value of probiotics in prevention and treatment.Methods A total of 60 children with lower respiratory tract infections who were routinely treated by antibiotics were divided into a control group and an observation group based on the random numerical table,with 30 cases in each group.The control group took conventional intervention,and the observation group took probiotic intervention.Both groups were compared in terms of occurrence of AAD at different times(1,3 and 6d)of antibiotic application,the number of Lactobacillus and diarrhea index score after the intervention.Results After 1,3 and 6d of antibiotic application,the incidence of AAD in children in the observation group were 3.33%,10.00%and 16.67%,which were lower than 23.33%,33.33%and 46.67%in the control group,and the differences were statistically significant(P<0.05).At 3 and 6d of intervention,the number of Lactobacillus in children in the observation group were(7.92±1.05)and(8.03±1.20)logcopies/g,which were higher than(6.58±0.90)and(6.30±0.90)logcopies/g in the control group;the stool shape scores in the observation group were(0.95±0.15)and(0.95±0.15)points,and the mucus stool scores were(0.70±0.05)and(0.48±0.03)points,which were lower than(1.25±0.16),(0.96±0.12)points and(0.96±0.08),(0.72±0.05)points in the control group;the differences were all statistically significant(P<0.05).Conclusion AAD is common during children's antibiotic treatment.Probiotics can effectively improve the intestinal flora,actively prevent and treat AAD,and improve the safety of children during antibiotic treatment.
