戈舍瑞林联合米非司酮治疗子宫肌瘤的疗效观察

Observation of the efficacy of goserelin combined with mifepristone in the treatment of uterine fibroids

目的评价戈舍瑞林联合米非司酮治疗子宫肌瘤的临床效果。方法110例子宫肌瘤患者,随机分为对照组和观察组,每组55例。对照组患者采取米非司酮治疗,观察组患者采取戈舍瑞林联合米非司酮治疗。比较两组患者治疗效果、子宫体积、肌瘤体积及性激素[促卵泡生成激素(FSH)、雌二醇(E2)、黄体生成素(LH)]水平。结果观察组患者治疗总有效率为94.55%,显著高于对照组的76.36%,差异有统计学意义(P<0.05)。治疗前,两组患者子宫体积及肌瘤体积比较,差异无统计学意义(P>0.05);治疗后,观察组患者子宫体积(197.20±20.50)cm^(3)及肌瘤体积(32.78±9.03)cm^(3)均小于对照组的(255.05±23.50)、(47.60±9.60)cm3,差异有统计学意义(P<0.05)。治疗前,两组患者FSH、E2、LH水平比较,差异无统计学意义(P>0.05);治疗后,观察组FSH(12.50±2.50)U/L、E2(137.80±17.50)pmol/L、LH(11.30±2.50)mU/ml均低于对照组的(18.30±4.05)U/L、(202.02±35.50)pmol/L、(15.30±4.05)mU/ml,差异有统计学意义(P<0.05)。结论戈舍瑞林联合米非司酮治疗子宫肌瘤可以明显缩小子宫和肌瘤体积,改善性激素水平,治疗预后效果显著。

Objective To evaluate the clinical effect of goserelin combined with mifepristone in the treatment of uterine fibroids.Methods A total of 110 patients with uterine fibroids were randomly divided into a control group and an observation group,with 55 cases in each group.Patients in the control group were treated with mifepristone,and patients in the observation group were treated with goserelin combined with mifepristone.The therapeutic effect,uterine volume,fibroids volume and sex hormone[follicle stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH)]levels of the two groups were compared.Results The total effective rate of treatment for patients in the observation group was 94.55%,which was significantly higher than 76.36%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in uterine volume and fibroids volume between the two groups(P>0.05).After treatment,the uterine volume(197.20±20.50)cm^(3) and fibroids volume(32.78±9.03)cm^(3) of patients in the observation group were smaller than(255.05±23.50)and(47.60±9.60)cm^(3) in the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in the levels of FSH,E2 and LH between the two groups(P>0.05).After treatment,the FSH(12.50±2.50)U/L,E2(137.80±17.50)pmol/L,and LH(11.30±2.50)mU/ml in the observation group were lower than(18.30±4.05)U/L,(202.02±35.50)pmol/L,and(15.30±4.05)mU/ml in the control group,and the differences were statistically significant(P<0.05).Conclusion Goserelin combined with mifepristone for the treatment of uterine fibroids can significantly reduce the uterine volume and fibroids volume,improve the level of sex hormones,and have a significant prognostic effect.
