术前颈部功能锻炼对甲状腺手术患者术后康复的影响

Effect of preoperative neck functional exercise on postoperative rehabilitation of patients with thyroid surgery

目的探究术前颈部功能锻炼对甲状腺手术患者术后康复的影响。方法72例甲状腺手术患者,根据是否展开术前颈部功能锻炼分为参照组与研究组,各36例。参照组未开展术前颈部功能锻炼,研究组开展术前颈部功能锻炼。对比两组术前及术后1、3个月的肩关节功能评分,术后并发症发生情况,术后颈部僵硬、瘢痕挛缩发生情况。结果术后1、3个月,研究组疼痛、日常生活能力、肌力、肩关节活动范围评分分别为(12.03±1.65)、(14.96±1.87)、(19.45±1.65)、(36.96±1.24)分与(13.36±1.13)、(17.96±1.65)、(23.48±1.35)、(38.13±2.36)分,均高于参照组的(11.36±1.13)、(11.36±1.98)、(17.33±1.87)、(35.65±1.27)分与(12.39±2.17)、(16.36±1.89)、(22.36±2.05)、(37.21±1.33)分,差异有统计学意义(P<0.05)。研究组术后并发症发生率2.78%低于参照组的16.67%,差异有统计学意义(P<0.05)。研究组患者术后颈部僵硬发生率5.56%、瘢痕挛缩发生率2.78%低于参照组的22.22%、16.67%,差异有统计学意义(P<0.05)。结论术前颈部功能锻炼对改善甲状腺手术患者术后康复效果有积极临床价值。

Objective To investigate the effect of preoperative neck functional exercise on postoperative rehabilitation of patients with thyroid surgery.Methods A total of 72 patients undergoing thyroid surgery were divided into a reference group and a research group according to whether they had preoperative neck functional exercise,with 36 cases in each group.The reference group did not perform preoperative neck functional exercise,and the research group performed preoperative neck functional exercise.The shoulder function scores preoperatively and at 1 and 3 months postoperatively,the occurrence of postoperative complications,postoperative neck stiffness and scar contracture were compared between the two groups.Results At 1 and 3 months postoperatively,the pain score,activities of daily living score,muscle strength score,and shoulder range of motion score in the research group were(12.03±1.65),(14.96±1.87),(19.45±1.65),(36.96±1.24)points versus(13.36±1.13),(17.96±1.65),(23.48±1.35),(38.13±2.36)points,which were higher than(11.36±1.13),(11.36±1.98),(17.33±1.87),(35.65±1.27)points versus(12.39±2.17),(16.36±1.89),(22.36±2.05),(37.21±1.33)points in the reference group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the research group was 2.78%,which was lower than 16.67%in the reference group,and the difference was statistically significant(P<0.05).The incidence of postoperative neck stiffness scar contracture were 5.56%and 2.78%in the research group was,which were lower than 22.22%and 16.67%in the reference group,and the difference was statistically significant(P<0.05).Conclusion Preoperative neck functional exercise has positive clinical value in improving the postoperative rehabilitation effect of patients with thyroid surgery.
