经皮椎间孔镜椎间盘切除术治疗腰椎间盘突出症的疗效分析

Efficacy analysis of percutaneous transforaminal endoscopic discectomy on lumbar disc herniation

目的分析经皮椎间孔镜椎间盘切除术治疗腰椎间盘突出症的临床疗效。方法46例腰椎间盘突出症患者,利用红绿双色球法分为对照组和研究组,各23例。对照组采用传统椎板开窗手术治疗,研究组采用经皮椎间孔镜椎间盘切除术治疗。比较两组手术情况及腰腿疼痛程度、腰椎功能恢复情况。结果研究组切口长度(0.92±0.11)cm与术后住院时间(5.11±0.24)d均短于对照组的(4.56±0.34)cm、(9.85±1.03)d,术中出血量(24.58±3.12)ml少于对照组的(72.89±5.37)ml,差异具有统计学意义(P<0.05)。研究组Oswestry功能障碍指数问卷表(ODI)评分(12.30±2.37)分、改良日本骨科协会腰痛评分表(M-JOA)评分(8.41±2.65)分均低于对照组的(14.56±2.70)、(12.50±2.86)分,差异均有统计学意义(P<0.05)。结论经皮椎间孔镜椎间盘切除术治疗腰椎间盘突出症效果显著,患者腰腿疼痛程度轻,腰椎功能恢复良好,临床可进一步推广运用。

Objective To analyze the clinical efficacy of percutaneous transforaminal endoscopic discectomy on lumbar disc herniation.Methods A total of 46 patients with lumbar disc herniation were divided into a control group and a research group by red and green ball method,with 23 cases in each group.The control group was treated by conventional laminectomy,and the research group was treated by percutaneous transforaminal endoscopic discectomy.The surgical condition,degree of lumbar pain and recovery of lumbar spine function were compared between the two groups.Results The incision length(0.92±0.11)cm and postoperative hospital stay(5.11±0.24)d in the research group were shorter than(4.56±0.34)cm and(9.85±1.03)d in the control group;the intraoperative blood loss(24.58±3.12)ml in the research group was less than(72.89±5.37)ml in the control group;the differences were statistically significant(P<0.05).The Oswestry Dysfunction Index Questionnaire(ODI)score(12.30±2.37)points and the Modified Japanese Orthopaedic Association(M-JOA)score(8.41±2.65)points in the research group were lower than(14.56±2.70)and(12.50±2.86)points in the control group,and the differences were statistically significant(P<0.05).Conclusion Percutaneous transforaminal endoscopic discectomy is effective for lumbar disc herniation,and the patients have mild back and leg pain and good recovery of lumbar spine function,and can be further popularized in clinic.
