低分子肝素联合期待治疗在重度子痫前期中的应用价值

Application value of low molecular weight heparin combined with expectant treatment in severe preeclampsia

目的研究低分子肝素联合期待治疗在重度子痫前期中的应用价值。方法选取2019年1月至2020年1月在本院就诊的148例重度子痫前期患者,按照治疗方法的不同将其分为对照组(74例,行期待治疗)和观察组(74例,在对照组基础上行低分子肝素治疗)。比较两组的治疗效果。结果治疗后,观察组的纤维蛋白原(FIB)、D-二聚体(D-D)水平低于对照组,凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)长于对照组(P<0.05)。治疗后,观察组的舒张压、收缩压、24h尿蛋白定量低于对照组(P<0.05)。观察组的终止妊娠时间、孕周延长时间长于对照组,新生儿出生体重大于对照组(P<0.05)。观察组的胎盘早剥、剖宫产发生率低于对照组(P<0.05)。观察组的早产、胎儿窘迫、新生儿窒息、肺透明膜病变发生率低于对照组(P<0.05)。结论低分子肝素联合期待治疗在重度子痫前期患者中的应用效果显著,有利于改善凝血功能指标。

Objective To study the application value of low molecular weight heparin combined with expectant treatment in severe preeclampsia.Methods A total of 148 patients with severe preeclampsia who were treated in our hospital from January 2019 to January 2020 were selected and divided into control group(74 cases,treated with expectant treatment)and observation group(74 cases,treated with low molecular weight heparin on the basis of the control group)according to different treatment methods.The therapeutic effects of the two groups were compared.Results After treatment,the levels of fibrinogen(FIB)and D-dimer(D-D)in the observation group were lower than those in the control group,and thrombin time(TT),prothrombin time(PT)and activated partial thromboplastin time(APTT)were longer than those in the control group(P<0.05).After treatment,the diastolic blood pressure,systolic blood pressure and 24 h urinary protein in the observation group were lower than those in the control group(P<0.05).The time of termination of pregnancy and the prolongation of gestational age in the observation group were longer than those in the control group,and the birth weight of the newborn was greater than that in the control group(P<0.05).The incidences of placental abruption and cesarean section in the observation group were lower than those in the control group(P<0.05).The incidences of premature delivery,fetal distress,neonatal asphyxia and hyaline membrane disease in the observation group were lower than those in the control group(P<0.05).Conclusion Low molecular weight heparin combined with expectant treatment in patients with severe preeclampsia has remarkable application effect,which is conducive to improving coagulation function indexes.
