基于流程管理法的院前急救护理联合体位护理对急性心肌梗死患者急救效率、心肌耗氧量及心脏电生理的影响

Effects of pre-hospital emergency care based on process management method combined with posture care on emergency efficiency,myocardial oxygen consumption and cardiac electrophysiology in patients with acute myocardial infarction

目的探讨基于流程管理法的院前急救护理联合体位护理对急性心肌梗死患者急救效率、心肌耗氧量(D-P)及心脏电生理的影响。方法选取2019年4月至2021年4月我院急诊科收治的68例急性心肌梗死患者作为研究对象,根据护理方式不同将其分为对照组和观察组,各34例。对照组采用常规院前急救护理,观察组采用基于流程管理法的院前急救护理联合体位护理。比较两组的护理效果。结果观察组的出诊反应时间、院前急救时间、急诊入院时间及住院时间均短于对照组(P<0.05)。平卧位(T0)、调整至30°半卧位(T_(1))时,两组的D-P无明显差异(P>0.05);半卧位1min(T_(2))、半卧位5min(T_(3))、半卧位10min(T_(4))时,观察组的D-P小于对照组(P<0.05)。T0~T_(2)、T_(2)~T_(3)、T_(3)~T_(4)时,观察组的QT离散度(QTd)、校正后的QT离散度(QTcd)低于对照组(P<0.05)。结论基于流程管理法的院前急救护理联合体位护理应用于急性心肌梗死患者中有助于提高急救效率,改善D-P、心脏电生理,值得推广。

Objective To explore the effects of pre-hospital emergency care based on process management method combined with posture care on emergency efficiency,myocardial oxygen consumption(D-P)and cardiac electrophysiology in patients with acute myocardial infarction.Methods Sixty-eight patients with acute myocardial infarction admitted in the emergency department of our hospital from April 2019 to April 2021 were selected as the research objects and divided into control group and observation group according to different nursing methods,with 34 cases in each group.The control group was given conventional pre-hospital emergency care,while the observation group was give pre-hospital emergency care based on process management method combined with posture care.The nursing effects of the two groups were compared.Results The response time of visit,pre-hospital emergency time,emergency admission time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in D-P between the two groups at prostrate position(T0)and adjust to 30°semi-decumbent position(T_(1))(P>0.05);the D-P in the observation group was smaller than that in the control group at 1 min of semi-decumbent position(T_(2)),5 min of semi-decumbent position(T_(3))and 10 min of semi-decumbent position(T_(4))(P<0.05).The QT dispersion(QTd)and corrected QT dispersion(QTcd)of the observation group were lower than those of the control group at T0-T_(2),T_(2)-T_(3)and T_(3)-T_(4)(P<0.05).Conclusion Pre-hospital emergency care based on process management method combined with posture care applied in patients with acute myocardial infarction is helpful to improve emergency efficiency,improve D-P and cardiac electrophysiology,which is worthy of promotion.
