提高经颈静脉肝内门体分流术中门静脉穿刺成功率的引导方法及研究进展

Guiding methods of improving the success rate of portal vein puncture in transjugular intrahepatic portosystemic shunt and its research progress

经颈静脉肝内门体分流术(TIPS)实施难点在于通过肝静脉成功穿刺门静脉。为了更好地显示门静脉及其分支,确定适当的穿刺点,减少穿刺次数,目前已报道多种提高术中门静脉穿刺成功率的方法,如影像融合技术、二氧化碳楔入肝静脉造影、超声引导等。该文将对这些提高TIPS术中门静脉穿刺成功率的导引方法及其研究进展进行综述。

The key difficulty of performing transjugular intrahepatic portosystemic shunt(TIPS)is the successful completion of puncturing the portal vein through the hepatic vein.In order to better display the portal vein and its branches,determine the optimal puncture point and reduce the times of puncturing,several methods used for improving the success rate of intraoperative portal vein puncture have been reported,such as image fusion technology,carbon dioxide wedging hepatic venography,ultrasound guidance,etc.This paper aims to review these guiding methods used for improving the success rate of portal vein puncture in TIPS,and to introduce the recent advances in these guiding methods.(J Intervent Radiol,2022,31:1111-1115)
