间歇持续鼻饲法对预防脑瘫伴有吞咽障碍患儿肺部感染及营养水平的效果观察

Effect of intermittent continuous nasal feeding on prevention of pulmonary infection and nutritional level in children with cerebral palsy complicated with dysphagia

目的:探究间歇持续鼻饲法应用于脑瘫伴有吞咽障碍患儿的营养干预效果及对预防肺部感染的作用。方法:选取我科2020年8月至2021年9月期间109例脑瘫伴有吞咽障碍患儿,随机分为观察组55例给予间歇持续鼻饲输注给予营养支持,对照组54例给予间歇鼻饲注入给予营养支持。2 m后,对比两组患儿肺部感染发生率,实验室营养指标:干预前后血红蛋白、血清总蛋白及白蛋白水平;吞咽功能:干预前后吞咽障碍调查评分;激素水平:干预前后生长抑素、胃泌素指标。结果:干预后观察组肺部感染发生率、吞咽障碍调查评分均低于对照组,血红蛋白、总蛋白、白蛋白、生长抑素、胃泌素水平高于对照组,有统计学意义(P<0.05)。结论:间歇持续鼻饲法营养支持方案对脑瘫伴有吞咽障碍患儿干预,可有效改善胃肠功能和营养状态,降低肺部感染发生情况,促进吞咽功能得以恢复。

Objective:To explore the effect of intermittent continuous nasal feeding method in nutritional intervention of children with cerebral palsy complicated with dysphagia and its influence on prevention of pulmonary infection,hormone levels and nutritional indicators.Methods:A total of 109 children with cerebral palsy complicated with dysphagia in our department from August 2020 to September 2021 were randomly selected and grouped.55 cases in the observation group were given intermittent continuous nasal feeding infusion for nutritional support,and 54 cases in the control group were given intermittent nasal feeding injection for nutritional support.Two months later,the incidence of pulmonary infection,laboratory nutrition indicators:hemoglobin,serum total protein,albumin before and after intervention;Swallowing function:Swallowing disorder survey score before and after intervention;Hormone levels:somatostatin and gastrin indexes before and after intervention.Results:After intervention,the incidence of pulmonary infection and the survey score of dysphagia in the observation group were lower than those in the control group,and the levels of hemoglobin,total protein,albumin,somatostatin and gastrin in the observation group were higher than those in the control group,with statistical significance(P<0.05).Conclusion:Intermittent continuous nasal feeding nutritional support program for children with cerebral palsy complicated with dysphagia can effectively improve gastrointestinal function and nutritional status,reduce the incidence of pulmonary infection,and promote the recovery of swallowing function.
