相依关系在脑卒中照顾者照顾负担与积极感受中的调节作用

Moderating effect of interdependent relationship on care burden and positive feelings during the caregiving process of stroke caregivers

目的探讨脑卒中照顾者相依关系在照顾负担与积极感受中的调节作用,为促进脑卒中照顾者在照顾过程中的身心健康提供新视角。方法采用方便抽样法选取牡丹江医学院附属红旗医院2020年10月至2021年7月间神经内科230名脑卒中照顾者,分别采用一般资料调查表、照顾负担量表、相依关系量表、积极感受量表进行数据收集问卷调查,采用SPSS 25.0进行统计分析,采用描述性统计方法分析脑卒中患者照顾者一般资料调查表、相依关系量表、照顾负担量表、积极感受量表的得分情况;应用Spearman相关性分析法分析脑卒中患者照顾者相依关系与照顾负担、积极感受的相关性;应用分层回归分析、Johnson-Neyman法进行简单斜率检验分析相依关系对照顾负担和积极感受的调节作用。结果脑卒中照顾者相依关系得分为31(20,43)分,照顾负担得分为36(25,46)分,积极感受得分为(30.92±8.31)分。Speaiman相关分析结果显示:脑卒中患者照顾者相依关系和照顾负担为负相关(r=-0.750,P<0.05),相依关系和积极感受呈正相关(r=0.514,P<0.05),照顾负担和积极感受呈负相关(r=-0.331,P<0.05);简单斜率检验分析结果显示:当相依关系得分在36.25分以上时,相依关系在脑卒中患者照顾者照顾负担及积极感受之间起调节作用。结论脑卒中患者照顾者相依关系处于较低水平,照顾负担处于轻度水平,积极感受处于中等偏高水平,脑卒中患者照顾者相依关系与照顾负担呈正相关,与积极感受呈负相关,照顾负担与积极感受呈负相关,高水平的相依关系在照顾负担与积极感受间起调节作用。因此,提示医务人员可通过提高照顾者相依关系来减轻照顾过程中的照顾负担,提高积极感受,促进其身心健康。

Objective To explore the moderating effect of the interdependence relationship on care burden and positive feelings during the caregiving process of stroke caregivers,and to provide a new perspective for promoting the physical and mental health of stroke caregivers during the care process.Methods The convenience sampling method was used to select 230 stroke caregivers from the Department of Neurology,Hongqi Hospital affiliated to Mudanjiang Medical College,from October 2020 to July 2021.The questionnaire was used for data collection,SPSS 25.0 was used for statistical analysis,and descriptive statistical methods were used to analyze the scores of the general information questionnaire,the dependency relationship scale,the care burden scale,and the positive feeling scale for stroke patients.Spearman correlation analysis was used to analyze the correlation between caregiver dependency,care burden and positive feelings of stroke patients.Hierarchical regression analysis and Johnson-Neyman method were used to conduct simple slope test to analyze the moderating effect of dependency on care burden and positive feelings.Results The caregiver’s dependency relationship score was 31(20,43),the care burden was 36(25,46),and the positive feeling score was(30.92±8.31).The results of Spearman correlation analysis showed that the dependency relationship and care burden of caregivers of stroke patients were negatively correlated(r=-0.750,P<0.05),and the dependency relationship was positively correlated with positive feelings(r=0.514,P<0.05).Positive feelings were negatively correlated(r=-0.331,P<0.05).The results of simple slope test analysis showed that when the score of dependency relationship was above 36.25,the interdependent relationship had moderating effect on care burden and positive feelings of caregivers of stroke patients.Conclusion The caregiver dependence of stroke patients is at a low level,the care burden is at a mild level,and the positive feeling is at a moderately high level.The caregiver dependence of stroke patients is positively correlated with care burden,and negatively correlated with positive feelings.Burden was negatively correlated with positive feelings,and a high level of dependency played a moderating role between caregiving burden and positive feelings.Therefore,it is suggested that medical staff can reduce the burden of care in the care process by improving the caregiver’s interdependence relationship,improve positive feelings,and promote their physical and mental health.
