远程心电网络在冠心病患者病情进展中的监测作用分析 被引量:2

摘要 目的 探讨远程心电网络在冠心病患者病情进展中的监测作用。方法 选取2019年3月—2020年2月湖南省脑科医院收治的200例冠心病患者,应用随机数字表法分为两组,各100例。对照组采用动态心电图监测,观察组采用远程心电网络监测。比较两组心电波形基线、心电各波形振幅、各波形时限、设备预警事件心电图符合率、不同时间段内检出心肌缺血情况、来院抢救病例就诊等待时间、治疗前费用、就医满意度视觉模拟评分法(VAS)评分。结果 两组心电波性基线比较,差异无统计学意义(P>0.05);两组同一时间点所测量的T波、P波、QRS波群振幅比较,差异无统计学意义(P>0.05);两组同一时间点Q-T间期、T时限、QRS时限、P-R间期、P波时限比较,差异无统计学意义(P>0.05);两组检出ST段改变、心脏传导阻滞、心房颤动、室性心动过速、窦性心动过缓、窦性心动过速情况比较,差异无统计学意义(P>0.05);两组不同时间段内对心肌缺血检出率比较,差异无统计学意义(P>0.05);观察组就诊等待时间、治疗前费用低于对照组,就医满意度评分高于对照组(P<0.05)。结论 远程心电网络心电图判读分析结果与动态心电图相似,具有临床应用的可靠性与准确性,且在自适应分析诊断、预警、实时监测等方面具有明显应用优势,可作为冠心病患者病情进展的监测工具,在院前急救、院外诊断与随访管理中有应用价值。 Objective To explore the role of remote ECG network in monitoring the progress of patients with coronary heart disease.Methods A total of 200 patients with coronary heart disease admitted to Hunan Brain Hospital from March 2019 to February 2020 were selected and divided into two groups using a random number table method,with100 cases in each group.The control group was monitored by Holter,and the observation group was monitored by remote ECG network.The baseline of the ECG waveform,the amplitude of each ECG waveform,the time limit of each waveform,the ECG coincidence rate of the equipment warning event,the detection of myocardial ischemia in different time periods,the waiting time of the hospital for rescue cases,the pre-treatment cost,and the satisfaction of the doctor Visual analogue scoring(VAS) scoring were compared between the two groups.Results There was no statistically significant difference between the two groups of ECG baselines(P>0.05);the comparison of the amplitudes of the T wave,P wave,and QRS complexes measured at the same time point between the two groups was not statistically significant(P>0.05);Comparison of QT interval,T time limit,QRS time limit,PR interval,and P wave time limit between the two groups at the same time point was not statistically significant(P>0.05);ST segment changes,heart block,and atrium were detected in the two groups There was no significant difference in fibrillation,ventricular tachycardia,sinus bradycardia,and sinus tachycardia(P>0.05);there was no difference in the detection rate of myocardial ischemia between the two groups in different time periods Statistically significant(P>0.05);the observation group’s waiting time and pre-treatment expenses were lower than those of the control group,and the medical satisfaction score was higher than that of the control group(P<0.05).Conclusion The results of remote ECG network interpretation and analysis are similar to those of Holter ECG.It has the reliability and accuracy of clinical application,and has obvious application advantages in adaptive analysis and diagnosis,early warning,and real-time monitoring.It can be used as an indicator of the progress of patients with coronary heart disease.Monitoring tools have shown application value in pre-hospital emergency,out-of-hospital diagnosis and follow-up management.
