早期预警分级联合急诊护理绿色通道对急性重症脑卒中患者的效果

Effects of modified early warning score combined with emergency nursing green channel on patients with acute severe stroke

目的 研究早期预警分级联合急诊护理绿色通道对急性重症脑卒中患者的效果。方法 选取2019年5月—2021年3月万宁市人民医院重症监护室92例急性重症脑卒中患者,按急诊救治模式分为两组:对照组43例,研究组49例。研究组实施早期预警分级联合急诊护理绿色通道模式抢救,对照组单纯实施急诊护理绿色通道干预。比较两组急诊分诊、救治与住院时间、转运意外情况,疗效,生活质量与并发症发生率。结果 研究组分诊、急诊抢救与住院时间均短于对照组,转运意外发生率低于对照组(P<0.05)。对照组治愈13例、显效21例、无效9例,研究组分别为18例、28例、3例,研究组有效率93.88%高于对照组79.07%(P<0.05)。治疗后研究组生理、心理与社会功能、环境支持评分均高于对照组(P<0.05)。对照组发生6例神经功能后遗症、3例泌尿系统感染、4例肺部感染,研究组分别发生1例、0例、1例,研究组并发症发生率低于对照组(P<0.05)。结论 早期预警分级联合急诊护理绿色通道能缩短急性重症脑卒中患者救治时间,提高疗效及生活质量,减少并发症发生情况,促进康复,推荐使用。

Objective To investigate the effects of modified early warning score(MEWS) combined with emergency nursing green channel on patients with acute severe stroke.Methods Ninety-two patients with acute severe stroke were hospitalized and randomly divided into 2 demographically and clinically matched 2 groups according to the rescue model:control group(n=43) received through emergency nursing green channel,and study group(n=49) received through emergency nursing green channel and undergoing MEWS assessment in addition.The emergency triage time,rescue time,hospitalization time,transportation accidents,efficacy,quality of life,and incidence of complications were compared between the two groups.Results The emergency triage time,rescue time and hospitalization time of the study group were all significantly shorter than those of the control group(all P<0.05).The rate of transportation accidents of the study group was 1%,significantly lower than that of the control group(8%,P<0.05).The total efficacy rate of the study group was 93.88%,significantly higher than that of the control group(79.07%,P<0.05).The quality of life scores in the domains of physical,psychological,and social functions,and in environmental support of the study group were all significantly higher than those of the control group(all P<0.05).There were 6 cases of neurological sequelae,3 cases of urinary system infection and 4 cases of pulmonary infection in the control group,while there were only 1 case of neurological sequelae and 1 case of pulmonary infection respectively in the study group.The complication rate of the study group was significantly lower than that of the control group(P<0.05).Conclusion Application of MEWS combined with emergency nursing green channel for patients with acute severe stroke effectively shortens the rescue time,improves the quality of life,reduces the complication rate,and promotes the recovery.
