十二指肠低度炎症在功能性消化不良中的研究进展

Research progress of duodenal micro-inflammation in functional dyspepsia

功能性消化不良是最常见的功能性胃肠疾病之一,其发病与胃-十二指肠运动障碍、内脏超敏反应、免疫激活、脑-肠轴异常、胃肠道微生物群失调和遗传易感性等机制相关。十二指肠在胃-十二指肠功能的控制和协调中起着重要的作用,十二指肠是功能性消化不良病理生理学的关键区域,十二指肠黏膜屏障损伤和低度炎症会引起机体痛觉过敏,降低神经肌肉细胞功能及免疫细胞活性;同时十二指肠低度炎症会破坏十二指肠上皮屏障,引起十二指肠屏障缺损,肠内容物包括十二指肠酸、胆汁、微生物群和饮食因素,而导致消化不良症状。该文主要探讨十二指肠低度炎症在功能性消化不良发病中的病理机制,以期为功能性消化不良的针对性治疗提供新思路。

Functional dyspepsia is one of the most common functional gastrointestinal diseases,and its pathogenesis is related to gastroduodenal motility disorder,visceral hypersensitivity,immune activation,brain-gut axis disorder,gastrointestinal microbiome disorder and genetic susceptibility.Duodenum plays an important role in the control and coordination of gastroduodenal function.Duodenum is a crucial area of the pathophysiology of functional dyspepsia.Damage to the duodenal mucosal barrier and micro-inflammation can cause hyperalgesia,reduce neuromuscular cell function and immune cell activity,and lead to dyspepsia.Intestinal contents that cause duodenal barrier defects include duodenal acid,bile,microbiota and dietary factors.This study aimed to explore the pathological mechanism of duodenal micro-inflammation in the pathogenesis of functional dyspepsia,to provide new ideas for the targeted treatment of functional dyspepsia.
