收缩力在非实性结节影像良恶性鉴别中的应用分析

摘要 目的探讨非实性结节良恶性影像特征表现,评估收缩力在腺癌影像诊断中的应用价值。方法选取我院CT检出的纯磨玻璃结节和混合磨玻璃结节共计104例,经病理及临床检查确诊为肺腺癌患者87例、良性结节患者17例,对比分析两组患者毛刺征、胸膜牵拉征、胸膜凹陷征及结节内部血管形态改变的显示率;同时将肺腺癌结节按照病理分为原位腺癌(AIS)组、微浸润性腺癌(MIA)组及浸润性腺癌(IAC)组,分别评估三组收缩力相关表现。结果腺癌结节收缩力影像表现显示率均显著高于良性结节组(P<0.05);AIS组结节收缩力低于MIA和IAC组,差异有统计学意义(P<0.05),IAC组结节收缩力表现略高于MIA组,但差异无明显统计学意义(P>0.05)。结论非实性结节收缩力的CT影像特征表现有助于良恶性结节的鉴别,值得在结节定性诊断中加以评估。 Objective To retrospectively analyze the imaging features of benign and malignant non solid nodules and to evaluate the application value of contractility in the imaging diagnosis of adenocarcinoma.Methods 107 cases of pure ground glass nodules and mixed ground glass nodules detected by CT in our hospital were selected.87 cases of lung adenocarcinoma and 17 cases of benign nodules were diagnosed by pathology and clinical examination.The display rates of prick sign,pleural traction sign,pleural indentation sign and vascular morphological changes inside the nodules in the two groups were compared and analyzed.At the same time,the lung adenocarcinoma nodules were divided into two groups according to pathology.The expression of contractility was evaluated in three groups:adenocarcinoma in situ(AIS),microinvasive adenocarcinoma(MIA)and invasive adenocarcinoma(IAC).Results The imaging findings of contractile force of adenocarcinoma nodules were significantly higher than that of benign nodules(P<0.05);the contractile force of AIS group was lower than that of MIA and IAC groups(P<0.05),and the contractile force of IAC group was slightly higher than those of MIA group,but the difference was not statistically significant(P>0.05).Conclusion The CT imaging features of the contractility of non solid nodules are helpful in the differential diagnosis of benign and malignant nodules,which is worthy of evaluation in the qualitative diagnosis of nodules.
