摘要
目的 探讨评估DCE-MRI半定量参数在同时累及宫体宫颈的宫颈癌与子宫内膜癌中的鉴别诊断价值。方法 选取81例经活检或术后病理证实的同时累及宫体宫颈的宫颈癌/子宫内膜癌患者,其中48例为宫颈癌,33例为子宫内膜癌,分析两种疾病的时间-信号强度曲线(TIC)类型、半定量参数(峰值时间、斜率值、强化率、廓清率)。结果 宫颈癌的曲线斜率值、强化率高于子宫内膜癌(P<0.05);宫颈癌的廓清率小于子宫内膜癌(P<0.05);宫颈癌和子宫内膜癌的曲线类型、峰值时间差异无统计学意义。结论 DCE-MRI半定量参数可用于同时累及宫体宫颈的宫颈癌与子宫内膜癌的鉴别诊断,宫颈癌的曲线斜率值、强化率高于子宫内膜癌,宫颈癌的廓清率小于子宫内膜癌。
Objective To evaluate the value of DCE-MRI semi-quantitative parameters in the differential diagnosis of cervical cancer and endometrial cancer involving uterine body and cervix at the same time. Methods 81 patients with cervical cancer/endometrial cancer confirmed by biopsy or postoperative pathology were included in this study, including 48 cases of cervical cancer and 33 cases of endometrial cancer. The type of time signal intensity curve(TIC) and semi-quantitative parameters(TTP,MSI, SER, WR) of the two diseases were analyzed. Results The curve slope and enhancement rate of cervical cancer were higher than those of endometrial cancer(P<0.05);the clearance rate of cervical cancer was lower than that of endometrial cancer(P<0.05);the curve type and peak time of cervical cancer and endometrial cancer were not statistically significant. Conclusion DCE-MRI semi-quantitative parameters can be used for the differential diagnosis of cervical cancer and endometrial cancer involving the uterine body and cervix at the same time. The curve slope value and enhancement rate of cervical cancer are higher than those of endometrial cancer, and the clearance rate of cervical cancer is lower than that of endometrial cancer.
