左炔诺孕酮宫内节育系统联合去氧孕烯炔雌醇片对排卵障碍性异常子宫出血患者性激素水平的影响

Effects of LNG-IUS Combined with Desogestreland Ethinylestradiol Tablets on Sex Hormone Levels in Patients with Abnormal Uterine Bleeding Due to Ovulation Disorder

目的:探讨左炔诺孕酮宫内节育系统(LNG-IUS)联合去氧孕烯炔雌醇片对排卵障碍性异常子宫出血患者的疗效。方法:金溪县妇幼保健计生服务中心是本次研究的病患来源,共计纳入排卵障碍性异常子宫出血患者80例,所选患者的治疗时段是2019年1月至2021年8月,以随机数字表法分为对照组、观察组,每组各40例。对照组采用去氧孕烯炔雌醇片治疗,观察组在对照组的基础上联合LNG-IUS治疗,均持续治疗3个月。评估两组患者的临床疗效;比较两组患者子宫内膜厚度、月经失血图(PBAC)评分、血红蛋白(Hb)、雌二醇(E2)、促黄体素(LH)及卵泡刺激素(FSH)水平变化;记录两组患者治疗期间不良反应发生率。结果:两组患者总有效率、不良反应发生率差异无统计学意义(P>0.05);观察组排卵障碍性异常子宫出血患者在治疗3个月后,子宫内膜厚度、PBAC评分、E2、LH、FSH、水平较对照组明显下降,Hb水平上升(P<0.05)。结论:LNG-IUS联合去氧孕烯炔雌醇片治疗排卵障碍性异常子宫出血患者的临床疗效较好,可调节患者性激素水平,降低患者出血量,促进患者快速恢复。

Objective:To investigate the value of levonorgestrel intrauterine system(LNG-IUS)combined with desogestreland ethinylestradiol tablets in patients with abnormal uterine bleeding due to ovulation disorder.Methods:From January 2019 to August 2021,80 patients with abnormal uterine bleeding due to ovulation disorder in Jinxi County Maternal and Child Health and Family Planning Service Center were selected and randomly divided into control group and observation group with 40 patients in each group.The control group was treated with desogestreland ethinylestradiol tablets,and the observation group was treated with LNG-IUS on the basis of the control group,and the treatment lasted for 3 months.The clinical efficacy of the two groups was evaluated.The endometrial thickness,pictorial blood loss assessment chart(PBAC)score,hemoglobin(Hb),estradiol(E2),luteinizing hormone(LH)and follicle-stimulating hormone(FSH)levels were compared between the two groups.The incidence of adverse reactions during treatment was recorded in the two groups.Results:The total effective rate and incidence of adverse reactions in two groups showed no significant difference(P>0.05).After 3 months of treatment,the endometrial thickness,PBAC score,E2,LH and FSH were significantly lower,and the Hb level was significantly higher(P<0.05).Conclusions:LNG-IUS combined with desogestreland ethinylestradiol tablets has a good clinical effect in the treatment of patients with abnormal uterine bleeding due to ovulation disorder,which can regulate the level of sex hormones,reduce the amount of bleeding and promote the rapid recovery of patients.
