齐拉西酮联合奥氮平在精神分裂症患者中的应用效果

Effect of Ziprasidone combined with Olanzapine in patients with schizophrenia

目的研究精神分裂症患者行齐拉西酮联合奥氮平治疗的效果及对糖脂代谢的影响。方法选取枣庄市精神卫生中心2020年4月至2021年4月收治68例精神分裂症患者作为研究对象,采用随机数字表法将其分为对照组和研究组各34例,对照组采用奥氮平治疗、研究组采用奥氮平+齐拉西酮治疗,比较两组的糖脂代谢指标、血清因子、精神症状评分及临床疗效。结果用药前两组的糖脂代谢指标比较,差异无统计学意义(P>0.05);用药后研究组三酰甘油(TG)、总胆固醇(TC)、空腹血糖(FPG)及低密度脂蛋白胆固醇(LDL-C)指标低于对照组,差异有统计学意义(P<0.05)。用药前两组的血清因子指标比较,差异无统计学意义(P>0.05);用药后研究组脑源性神经营养因子(BDNF)、去甲肾上腺素(NE)、5-羟色胺(5-HT)指标高于对照组,肿瘤坏死因子-α(TNF-α)指标低于对照组,差异有统计学意义(P<0.05)。用药前两组的精神症状评分比较,差异无统计学意义(P>0.05);用药后研究组精神分裂症阴性与阳性症状量表(PANSS)评分低于对照组,差异有统计学意义(P<0.05)。研究组治疗有效率高于对照组,差异有统计学意义(P<0.05)。结论齐拉西酮联合奥氮平治疗精神分裂症可改善糖脂代谢能力、减缓炎症反应,调节血清因子并改善精神症状,增强疗效、值得推崇。

Objective To study the effect of Ziprasidone combined with Olanzapine in the treatment of schizophrenia and its influence on glucose and lipid metabolism.Methods Data of 68 patients with schizophrenia admitted to Zaozhuang Mental Health Center from April 2020 to April 2021 were selected as the research objects,and they were divided into the control group and the study group by the random number table method,with 34 cases in each group.The control group was treated with Olanzapine,and the study group was treated with Olanzapine+Ziprasidone.The glucose and lipid metabolism indexes,serum factors,psychiatric symptom scores and clinical efficacy were compared between the two groups.Results Before medication,there were no significant differences in glucose and lipid metabolism indexes between the two groups(P>0.05).After treatment,triglyceride(TG),total cholesterol(TC),fasting blood glucose(FPG)and low density lipoprotein cholesterol(LDL-C)in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in serum factor indexes between the two groups before medication(P>0.05).After treatment,the indexes of brain-derived neurotrophic factor(BDNF),norepinephrine(NE)and 5-hydroxytryptamine(5-HT)in the study group were higher than those in the control group,and the indexes of tumor necrosis factor-α(TNF-α)were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in psychiatric symptom scores between the two groups before medication(P>0.05).After treatment,the positive and negative symptoms scale(PANSS)score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Ziprasidone combined with Olanzapine in the treatment of schizophrenia can improve glucose and lipid metabolism,slow down inflammation,regulate serum factors and improve mental symptoms,and enhance the efficacy,which is worthy of praise.
