舒芬太尼鞘内注射在经皮椎间孔镜手术中的应用效果

Application effect of Sufentanil intrathecal injection in percutaneous foraminal surgery

目的探讨舒芬太尼鞘内注射在经皮椎间孔镜手术中的应用效果。方法选取2020年6月至2022年2月新余市人民医院麻醉科收治的60例经皮椎间孔镜手术患者为研究对象,按照随机抽签法分为对照组(30例)与观察组(30例),对照组采用1%利多卡因局部浸润麻醉,观察组采用舒芬太尼鞘内注射麻醉,比较两组患者的生命体征、麻醉效果与不良反应。结果观察组刺激神经根时(T2)、摘除髓核时(T3)、手术结束时(T4)的心率(HR)、平均动脉压(MAP)均低于对照组,差异有统计学意义(P<0.05);两组不同时刻的脉搏氧饱和度比较,差异无统计学意义(P>0.05);两组切皮时(T0)、穿刺定位时(T1)的HR、MAP比较,差异无统计学意义(P>0.05);T1、T2、T3、T4时,观察组镇痛评分低于对照组,镇静评分高于对照组,差异有统计学意义(P<0.05);两组患者的不良反应比较,差异无统计学意义(P>0.05)。结论在经皮椎间孔镜手术中采用舒芬太尼鞘内注射的效果显著,具有显著的镇痛作用,有效降低患者疼痛感,且镇静效果较满意,并不会影响患者生命体征,不良反应较少,具有较高的安全性,值得推广。

Objective To explore the effect of Sufentanil intrathecal injection in percutaneous foraminal surgery.Methods A total of 60 patients with percutaneous intervertebral foraminal surgery treated in the Department of Anesthesiology of Xinyu People's Hospital from June 2020 to February 2022 were selected as the research objects.They were divided into the control group(30 cases)and the observation group(30 cases)by random drawing lots.The control group was anesthetized with 1%Lidocaine local infiltration,and the observation group was anesthetized with Sufentanil intrathecal injection.The vital signs,anesthetic effects and adverse reactions of the two groups were compared.Results The heart rate(HR)and mean arterial pressure(MAP)of the observation group at the time of nerve root stimulation(T2),nucleus pulposus removal(T3)and the end of operation(T4)were lower than those of the control group,with statistically significant differences(P<0.05).There were no significant differences in pulse oxygen saturation between the two groups at different time(P>0.05).And there were no significant differences in HR and MAP between the two groups at the time of skin resection(T0)and puncture localization(T1)(P>0.05).At T1,T2,T3 and T4,the analgesic scores of the observation group were lower than those of the control group,and the sedation scores were higher than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The intrathecal injection of Sufentanil in percutaneous intervertebral foraminal surgery has a significant effect.It has a significant analgesic effect,effectively reduces the pain of patients,and has a satisfactory sedative effect,does not affect the vital signs of patients,has less adverse reactions,has a high safety,and is worthy of promotion.
