保留下颌骨及牙列术在舌癌根治术后患者中的应用研究

Study of Application Value of Mandible Retention and Dentitherapy in Patients After Radical Tongue Cancer

目的探讨舌癌根治术后应用保留下颌骨及牙列术治疗的效果。方法选取86例舌癌根治术后患者,采用随机数字表法分为2组,各43例。对照组术后行下颌骨矩形切除术,观察组术后行保留下颌骨及牙列术治疗,术后随访36个月。对2组临床效果、生活质量、复发率及死亡率进行比较。结果与对照组比较,观察组吞咽、发音、咬合正常及颜面外形良好率较高,华盛顿大学生活质量量表(UW-QOL)评分较高,差异有统计学意义(P<0.05)。2组复发率、死亡率比较,差异无统计学意义(P>0.05)。结论在舌癌根治术后患者中应用保留下颌骨及牙列术治疗效果较佳,既可有效根除肿瘤,又可保留患者良好的面部外形及吞咽、咬合、发音功能,有利于生活质量的提高,是一种理想的治疗方案。

Objective To investigate the effect of mandible retention and dentitherapy in patients after radical tongue cancer.Methods 86 tongue cancer patients were selected and divided into 2 groups of 43 patients.The control group underwent postoperative rectangular resection of the mandible,and the observation group underwent postoperative retained mandible and dentition,and was followed up for 36 months.The clinical effects,quality of life,relapse rate and mortality in the 2 groups were compared.Results Compared with the control group,the observation group had a higher rate of normal swallowing,pronunciation and occlusion,and the University of Washington quality of Life Scale(UW-QOL)had a higher score and a significant difference(P<0.05).The recurrence rate and mortality rate in the 2 groups were not significantly different(P>0.05).Conclusion Keeping mandible and dentition are better in patients after radical tongue cancer,which is an ideal treatment plan for the improvement of quality of life.
