三维超声血管血流参数对子宫内膜癌患者腹腔镜治疗预后的评估价值

Study on Three-dimensional Ultrasound Vascularity-flow Index in the Evaluation of Prognosis of Patients with Endometrial Cancer Treated by Laparoscopy

目的分析三维超声血管血流参数对子宫内膜癌(EC)患者腹腔镜治疗预后的评估价值。方法选择接受腹腔镜根治术治疗的160例EC患者为研究对象,术后随访24个月,将随访期间患者疾病复发情况作为预后观察指标并分组,分为预后不良组与预后良好组,调查并记录患者相关资料、随访期间最后一次检查时三维超声血管血流参数[平均灰阶值(MGV)、阻力指数(RI)、血管-血流指数(VFI)],分析三维超声血管血流参数对EC患者腹腔镜治疗预后的评估价值。结果腹腔镜治疗后随访24个月,160例患者中预后不良20例,占12.50%。预后不良组患者三维超声血管血流参数RI值小于预后良好组患者,VFI值大于预后良好组患者,差异有统计学意义(P<0.05);RI、VFI单独及联合检测评估EC患者腹腔镜治疗预后不良的AUC分别为0.850、0.878、0.893,均有一定评估价值,且当RI、VFI分别取0.435、4.840时,可获得最佳评估价值。结论三维超声血管血流参数对EC患者腹腔镜治疗预后具有一定评估价值。

Objective To analyze the evaluation value of three-dimensional ultrasound vascularity-flow index in prognosis of patients with endometrial cancer(EC)treated by laparoscopy.Methods 160 EC patients who received laparoscopic radical surgery were selected.The patients were followed up for 24 months.The disease recurrence during the follow-up period was used as a prognostic observation indicator and divided into 2 groups:poor prognosis group and good prognosis group,the relevant data of patients,three-dimensional ultrasound vascularity-flow index[mean gray scale value(MGV),resistance index(RI),vascularity-flow index(VFI)]at the last examination during follow-up were investigated and recorded,the evaluation value of three-dimensional ultrasound vascularity-flow index in the prognosis of patients with EC treated by laparoscopy was analyzed.Results Followed up for 24 months after laparoscopic treatment,20 patients of all 160 patients had poor prognosis,accounted for 12.50%;the RI value of three-dimensional ultrasound vascularity-flow index in patients with poor prognosis was lower than that in patients with good prognosis,and the VFI value was higher than that in patients with good prognosis,with statistical significant differences(P<0.05);the AUC of single and combined detection of RI and VFI in evaluating the poor prognosis of EC patients after laparoscopic treatment were 0.850,0.878 and 0.893 respectively,which had certain evaluation value,when RI and VFI were 0.435 and 4.840 respectively,the best evaluation value could be obtained.Conclusion Three dimensional ultrasound vascularity-flow index have certain evaluation value in the prognosis of patients with EC after laparoscopic treatment.
