多学科MDT诊疗模式在胶质母细胞瘤患者诊疗中的应用价值

Application value of multidisciplinary team pattern in diagnosis and treatment for glioblas⁃toma multiforme

目的:探讨多学科MDT(multidisciplinary team)诊疗模式在胶质母细胞瘤(glioblastoma multiforme,GBM)患者诊疗中的应用价值。方法:选择辽宁省肿瘤医院于2015年1月至2018年1月间253例在神经外科住院治疗的胶质母细胞瘤患者为研究对象,根据诊疗模式分为MDT组182例和非MDT组71例。比较两组患者一般信息、患者满意度、治疗前等待时间、无进展生存期(progression-free survival,PFS)及总生存期(overall survival,OS)等。结果:MDT组的患者满意度高于非MDT组,差异有统计学意义(P<0.01);MDT组治疗前等待时间短于非MDT组,分别为(18.20±4.26)天和(21.66±3.52)天,差异有统计学意义(P<0.05);MDT组和非MDT组无进展生存期PFS分别为10.42和9.72个月,差异无统计学意义;MDT组和非MDT组总生存期OS分别为19.11月和15.86个月,差异有统计学意义(P<0.05)。结论:多学科MDT模式为胶质母细胞瘤患者的最佳诊疗模式,缩短了治疗前等待时间,提高了患者的满意度,并一定程度上延长了患者的总生存期。

Objective:To investigate the application value of MDT in the diagnosis and treatment of patients with glioblastoma multiforme(GBM).Methods:253 GBM patients hospitalized in neurosurgery between January 2015 and January 2018 from Liaoning Cancer Hospital&Institute were selected.According to the pattern of diagnosis and treat⁃ment,they were divided into MDT group(n=182)and non-MDT group(n=71).Several information such as the general information,patients'satisfaction,wait time before treatment,progression-free survival(PFS)and overall survival(OS)was compared between the two groups.Results:Patients'satisfaction was higher in the MDT group than in the non-MDT group,and the difference was statistically significant(P<0.01).The waiting time before treatment was shorter in the MDT group than in the non-MDT group,for(18.20±4.26)days and(21.66±3.52)days re⁃spectively,and the difference was statistically significant(P<0.05).The PFS in the MDT group and non-MDT group was 10.42 and 9.72 months,respectively,and the difference was not statistically significant.The OS in MDT and non-MDT groups was 19.11 months and 15.86 months respectively,and the difference was statistically signifi⁃cant(P<0.05).Conclusion:MDT is the best pattern for diagnosis and treatment of GBM patients,shortening the waiting time before treatment,improving patients'satisfaction,besides improving their overall survival to a certain ex⁃tent.
