中医药治疗艾滋病疗效及安全性回顾性研究

Clinical efficacy and safety of traditional Chinese medicine in treatment of AIDS:A retrospective study

目的:分析湖南省中医药治疗艾滋病(AIDS)的疗效和安全性,为中西医协同治疗AIDS提供依据。方法:收集2013年9月至2018年9月中医药防治AIDS关爱项目湖南省项目点332例AIDS患者的临床资料,按照治疗方法不同分为治疗组与对照组。对照组145例采用高效抗逆转录病毒疗法(HAART)治疗,治疗组187例在对照组基础上进行中医分期论治。观察2组的CD4+T淋巴细胞绝对计数,症状体征总积分,肾功能[尿素氮(BUN)、血肌酐(SCr)],肝功能[天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)]及不良反应发生情况。结果:治疗后的第24、36个月2组CD4+T淋巴细胞计数、SCr水平组间比较,差异均有统计学意义(P<0.05或P<0.01)。治疗后第12、24、36个月2组症状体征总积分及AST、ALT水平组间比较,差异均有统计学意义(P<0.05或P<0.01)。不良反应发生率治疗组为24.06%(45/187),对照组为35.86%(52/145),2组比较,差异有统计学意义(P<0.05)。结论:中医药能够促进AIDS患者免疫重建,提高临床疗效,减少不良反应,具有较好的安全性。

Objective:To investigate the clinical efficacy and safety of traditional Chinese medicine in the treatment of AIDS in Hunan Province of China,and to provide a basis for the integrated traditional Chinese and Western medicine therapy for AIDS.Methods:Clinical data were collected from 332 AIDS patients who participated in the project of traditional Chinese medicine in the prevention and treatment of AIDS in Hunan Province from September 2013 to September 2018,and they were divided into treatment group and control group by different therapy.The 145 patients in the control group received highly active antiretroviral therapy(HAART),and the 187 patients in the treatment group received traditional Chinese medicine treatment by stage in addition to the treatment in the control group.The two groups were observed in terms of absolute CD4+T lymphocyte count,total score of symptoms and signs,renal function[blood urea nitrogen(BUN)and serum creatinine(SCr)],liver function[aspartate amin-otransferase(AST)and alanine aminotransferase(ALT)],and adverse reactions.Results:At 24 and 36 months after treatment,there were significant differences between the two groups in CD4+T lymphocyte count and SCr level(P<0.05 or P<0.01).At 12,24,and 36 months after treatment,there were significant differences between the two groups in the total score of symptoms and signs and the levels of AST and ALT(P<0.05 or P<0.01).There was a significant difference in the incidence rate of adverse reactions between the treatment group and the control group[24.06%(45/187)vs 35.86%(52/145),P<0.05].Conclusion:Traditional Chinese medicine can promote immune recovery,improve treatment outcome,and reduce adverse reactions in AIDS patients and has good safety.
