下肢动脉硬化闭塞症介入术后再狭窄的影响因素

Influencing Factors of Restenosis After Interventional Therapy in Lower Extremity Arteriosclerosis Obliterans

目的分析下肢动脉硬化闭塞症患者经介入治疗后发生再狭窄的影响因素。方法将河南省人民医院血管外科于2019年10月至2021年10月收治的40例患有下肢动脉硬化闭塞症经介入治疗后随访6个月内发生再狭窄的患者作为发生组纳入,另将同期随访未发生再狭窄的46例患者作为未发生组纳入。在此基础上充分收集两组患者的一般资料,包括性别、年龄、体质量指数(BMI)、病变部位、糖尿病、高血压、长期吸烟、高脂血症以及Rutherford分级等。将两组患者的相关信息进行同质对比并分析影响下肢动脉硬化闭塞症患者经介入治疗后出现再狭窄的影响因素。结果两组患者在糖尿病、高血压、长期吸烟、高脂血症方面差异有统计学意义(P<0.05);另对比分析其他资料差异无统计学意义(P>0.05);后经logistic回归分析,糖尿病、高血压、长期吸烟、高脂血症是影响患者术后再狭窄发生的独立危险因素(OR>1,P<0.05)。结论介入术治疗下肢动脉硬化闭塞症的复发率受糖尿病、高血压、长期吸烟、高脂血症影响,故可针对性制定出完善的干预方案以降低其术后再狭窄的发生率。

Objective To analyze the influencing factors of restenosis in patients with lower extremity arteriosclerosis obliterans after interventional therapy.Methods A total of 40 patients with lower extremity arteriosclerosis obliterans who were treated in the Department of Vascular Surgery,Henan Provincial People’s Hospital from October 2019 to October 2021 and had restenosis within 6 months after interventional treatment were included as the occurrence group,and 46 patients who did not have restenosis in the follow-up results of the same period were included as the non occurrence group.On this basis,the general data of patients in both groups were fully collected,including gender,age,body mass index(BMI),lesion location,diabetes,hypertension,long-term smoking,hyperlipidemia and Rutherford classification.The relevant information of patients in the two groups was compared homogeneously and the influencing factors of restenosis in patients with lower extremity arteriosclerosis obliterans after interventional treatment were analyzed.Results Compared diabetes mellitus,hypertension,long-term smoking and hyperlipidemia of patients in both groups,the difference was statistically significant(P<0.05).There was no statistical significant difference in other data(P>0.05).Logistic regression analysis showed that diabetes mellitus,hypertension,long-term smoking and hyperlipidemia were independent risk factors for restenosis in patients after operation(OR>1,P<0.05).Conclusion The recurrence rate of interventional treatment of lower extremity arteriosclerosis obliterans is affected by diabetes mellitus,hypertension,long-term smoking and hyperlipidemia,so a perfect intervention plan can be formulated to reduce the incidence of postoperative restenosis.
