晚期乳腺癌化疗患者生命意义感的影响因素

Influencing Factors of Life Meaning in Patients with Advanced Breast Cancer After Chemotherapy

目的观察晚期乳腺癌化疗患者生命意义感现状,并分析影响因素。方法选取2019年8月至2020年10月河南大学第一附属医院接收的68例晚期乳腺癌患者,化疗4个疗程时,用患者生命意义感依据生命意义感量表(PIL)进行评定,询问患者一般资料,并纳入可能的影响因素,分析可能影响晚期乳腺癌化疗患者生命意义感的影响因素。结果68例晚期乳腺癌患者中,生命意义感较低水平患者28例,占41.18%;生命意义感较低组受教育程度、应对方式、社会支持评定量表(SSRS)评分、创伤后成长评定量表(PTGI)评分与生命意义感较高组比较,差异有统计学意义(P<0.05);经logistic回归分析,结果显示,初中及以下受教育程度、消极应对是晚期乳腺癌化疗患者生命意义感较低的影响因素(OR>1,P<0.05);SSRS评分高、PTGI评分高是晚期乳腺癌化疗患者生命意义感较低的保护因素(OR<1,P<0.05)。结论晚期乳腺癌化疗患者生命意义感水平较低可能与初中及以下受教育程度、消极应对、SSRS评分低、PTGI评分低等因素有关。

Objective To observe the current situation of life meaning in patients with advanced breast cancer after chemotherapy,and analyze the influencing factors.Methods A total of 68 patients with advanced breast cancer in the First Affiliated Hospital of Henan University from October 2019 to August 2020 were selected.During the four courses of chemotherapy,life meaning of patients was assessed according to the presence of life meaning(PIL),the general data of patients were asked,the possible influencing factors were included,and the influencing factors that might affect life meaning of patients with advanced breast cancer undergoing chemotherapy were analyzed.Results Among 68 patients with advanced breast cancer,28 patients had lower meaning in life,accounting for 41.18%.Compared the education level,coping style,social support rate scale(SSRS)score and posttraumatic growth inventory(PTGI)score of the low meaning in life group with those of the high meaning in life group,the difference was statistically significant(P<0.05).The results of logistic regression analysis showed that,junior high school education,negative coping were the influencing factors of lower meaning in life in patients with advanced breast cancer after chemotherapy(OR>1,P<0.05),and high SSRS score and high PTGI score were the protective factors for lower meaning in life in patients with advanced breast cancer after chemotherapy(OR<1,P<0.05).Conclusion The lower level of meaning in life in patients with advanced breast cancer after chemotherapy may be related to low education level,passive coping style,low SSRS score and low PTGI score.
