非霍奇金淋巴瘤患者血清TK-1、IL-21与预后不良的相关性

摘要 目的观察血清胸苷激酶-1(TK-1)、白细胞介素-21(IL-21)在非霍奇金淋巴瘤(NHL)患者中表达水平,并分析二者与NHL患者临床预后的相关性。方法选取信阳职业技术学院附属医院2018年12月至2019年11月收治的102例NHL患者,全部患者均接受为期2 a随访,统计随访期间患者的生存情况(以病死作为终点事件)并进行分组,将随访期间病死患者纳入预防不良组,将随访期间存活患者纳入预后良好组。全部患者入院时均检测血清TK-1、IL-21水平,比较预后不良组、预后良好组一般资料以及血清TK-1、IL-21水平,分析血清TK-1、IL-21水平与NHL患者预后不良的相关性。结果经2 a随访结果显示,102例NHL患者中病死22例(21.57%),存活80例(78.43%),预后不良发生率为21.57%;全部患者生存时间为1~24个月,中位生存时间为21个月,平均(21.65±0.49)个月;预后不良组入院时血清TK-1水平高于预后良好组,入院时血清IL-21水平低于预后良好组,差异有统计学意义(P<0.05);经点二列相关性检验,结果显示,血清TK-1水平与NHL患者临床预后呈正相关(r=0.620,P<0.001),血清IL-21水平与NHL患者临床预后呈负相关(r=-0.554,P<0.001);经Cox回归分析,结果显示,入院时血清TK-1过表达、IL-21低表达均是NHL患者预后不良的风险因子(P<0.05)。结论血清TK-1、IL-21水平与NHL患者预后不良有一定关系,且血清TK-1过表达、IL-21低表达提示NHL患者发生预后不良的风险较高。 Objective To observe the expression levels of serum thymidine kinase-1(TK-1)and interleukin-21(IL-21)in patients with non-Hodgkin’s lymphoma(NHL),and to analyze the correlation between them and the clinical prognosis of patients with NHL.Methods A total of 102 patients with NHL treated in the Affiliated Hospital of Xinyang Vocational and Technical College from December 2018 to November 2019 were selected.All patients were followed up for 2 years.The survival of patients during the follow-up period(taking death as the end event)was counted and grouped.The patients who died from illness during the follow-up period were included in the poor prevention group and the patients who survived during the follow-up period were included in the good prognosis group.The levels of serum TK-1 and IL-21 were detected in all patients at admission.The baseline data of poor prognosis group and good prognosis group and the levels of serum TK-1 and IL-21 were compared.The correlation between the levels of serum TK-1 and IL-21 and the poor prognosis of NHL patients was analyzed.Results The results of 2-year follow-up showed that 22 cases(21.57%)died from illness and 80 cases(78.43%)survived in all 102 NHL patients.The incidence of poor prognosis was 21.57%.The survival time of all patients was 1-24 months,and the median survival time was 21 months,with an average of(21.65±0.49)months.The level of serum TK-1 in the poor prognosis group at admission was higher than that in the good prognosis group,and the level of serum IL-21 in the poor prognosis group at admission was lower than that in the good prognosis group,and the difference was statistically significant(P<0.05).Through the correlation test of point two columns,the results showed that the level of serum TK-1 was positively correlated with the clinical prognosis of patients with NHL(r=0.620,P<0.001),and the level of serum IL-21 was negatively correlated with the clinical prognosis of patients with NHL(r=-0.554,P<0.001).By Cox regression analysis,the results showed that overexpression of serum TK-1 and low expression of IL-21 were risk factors for poor prognosis in patients with NHL(P<0.05).Conclusion The levels of serum TK-1 and IL-21 are related to the poor prognosis of patients with NHL,and the overexpression of serum TK-1 and low expression of IL-21 suggest that patients with NHL have a higher risk of poor prognosis.
