身痛逐瘀汤加减治疗对脊柱胸腰段椎体骨折术后早期血液高凝状态的影响研究

The effects of the Shentong Zhuyu decoction on early postoperative blood hypercoagulability after thoracolumbar vertebral fractures

目的:观察身痛逐瘀汤加减治疗对脊柱胸腰段椎体骨折术后早期血液高凝状态的影响。方法:选取2018年2月-2020年11月接收的脊柱胸腰段椎体骨折术后患者80例,按照随机数字表法分为两组,各40例,两组均行经皮椎体后凸成形术治疗,对照组接受碳酸钙D3片辅助治疗,观察组行身痛逐瘀汤加减治疗,对比两组治疗效果、中医证候积分、血瘀证评分情况及骨密度情况。结果:观察组总有效率显著高于对照组,差异存在统计学意义(P<0.05)。治疗前,两组患者中医证候积分、血瘀证评分的差异不存在统计学意义(P>0.05)。治疗后,两组中医证候积分、血瘀证评分均下降,且观察组指标显著低于对照组,差异存在统计学意义(P<0.05)。对比两组患者术后3个月腰椎骨密度情况,观察组患者骨密度高于对照组(P<0.05)。结论:身痛逐瘀汤加减治疗对脊柱胸腰段椎体骨折术后早期血液高凝状态的疗效显著,可有效改善患者血瘀症状,安全性好,多加推广应用有助于提高治疗效果,减少不良反应的发生。

Objective:To observe the effect of the Shentong Zhuyu decoction(身痛逐瘀汤)on early postoperative blood hypercoagulability after thoracolumbar vertebral fractures.Methods:A total of 80 cases received from February 2018 to November 2020 were selected and randomly divided into two groups according to the random number table method,with 40 cases in each group.Both groups were given percutaneous kyphoplasty.The control group additionally received calcium carbonate D3 tablet adjuvant therapy,and the observation group was additionally treated with the Shentong Zhuyu decoction.Results:The total efficiency in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in TCM syndrome score and blood stasis syndrome score between the two groups,and P>0.05.After treatment,TCM syndrome scores and blood stasis syndrome scores in the two groups were all decreased,and those indexes in the observation group were significantly lower than those in the control group(P<0.05).The bone mineral density of the lumbar spine was compared between the two groups at 3 months after operation,and the bone mineral density in the observation group was higher than the control group(P<0.05).Conclusion:The Shentong Zhuyu decoction shows significant curative effects on early postoperative blood hypercoagulability after thoracolumbar vertebral fractures,can effectively ameliorate the symptoms of blood stasis of patients,with good safety.More promotion and application of this treatment method will help to improve the treatment effect,and reduce the occurrence of adverse reactions.
