摘要
目的初步探讨高原性心脏病的心脏磁共振(CMR)表现。方法回顾性分析2018年8月至2021年7月在本院经临床诊断为高原性心脏病的40例患者临床及CMR资料。CMR扫描序列主要为电影序列和T1加权成像(T1WI)、液体翻转恢复衰减序列(FLAIR)以及钆造影剂延迟增强(LGE)序列、纵向驰豫时间定量成像(T1mapping)。结果40例患者共累及右室52壁67段,其中前壁28例,下壁22例,侧壁12例,基底段13例,中间段37例,心尖段17例,另见累及室间隔前、后插入部26例;CMR表现为38例患者右室壁增厚、肌小梁增粗、紊乱和运动减慢,其中部分患者并有室壁抖动;32例患者伴有FLAIR序列信号增高和T1mapping值增高;36例LGE斑片或短条状延迟强化。结论高原性心脏病具有典型的影像特征,CMR可较为准确、客观地显示心脏结构、功能和病变范围,为心肌病的干预和治疗提供依据。
Objective To preliminarily explore the cardiovascular magnetic resonance(CMR) manifestations of high altitude heart disease. Methods A retrospective analysis of the clinical and CMR data of 40 patients with high altitude heart disease who were clinically diagnosed in our hospital from August 2018 to July 2021. CMR scanning sequence is mainly film sequence and T1WI, T2WI, LGE, T1mapping. Results A total of 67 segments of 52 walls of the right ventricle were involved in 40 patients(Table 2), including 28 anterior wall, 22 inferior wall, 12 lateral wall,13 basal segment, 37 intermediate segment, and 17 apical segment. Twenty-six cases of interventricular septum anterior and posterior insertion, CMR showed 38 patients with right ventricular wall thickening, trabecular thickening,disorder and movement Decrease, some patients have wall shaking, 32 patients have increased FLAIR sequence signal and increased T1mapping value, 36 patients have LGE patches or short strips of delayed enhancement.Conclusion High altitude heart disease has typical imaging features, and CMR can accurately and objectively display the cardiac structure, function, and lesion range, providing a basis for the intervention and treatment of cardiomyopathy.
