针灸镇痛对下肢骨折手术患者治疗效果及睡眠质量的影响研究

Study on the Effect of Acupuncture and Moxibustion Analgesia on the Treatment Effect and Sleep Quality of Patients Undergoing Lower Limb Fracture Surgery

目的:观察针灸镇痛对下肢骨折手术患者治疗效果及睡眠质量的影响。方法:选取2019年2月至2022年2月中国人民解放军联勤保障部队第九〇七医院骨科收治的下肢骨折患者200例作为研究对象,按照随机数字表法分为观察组和对照组,每组100例。对照组给予酮咯酸氨丁三醇进行镇痛治疗,观察组在对照组基础上联合给予针灸镇痛治疗,比较2组床活动时间、总住院时间;采用视觉模拟评分法(Visual Analogue Scale,VAS)比较2组患者术后疼痛情况,采用匹兹堡睡眠指数量表(Pittsburgh Sleep Quality Index,PSQI)比较2组患者睡眠质量的差异,同时比较2组患者术后并发症发生率。结果:干预后,观察组总住院时间、下床活动时间均短于对照组,观察组术后1 d、术后7 d VAS评分显著低于对照组,观察组术后1 d、术后7 d的PSQI评分显著低于对照组,观察组不良反应发生率显著低于对照组,2组比较差异均有统计学意义(均P<0.05)。结论:增加镇痛方法能够有效提高下肢骨折手术患者的治疗效果,改善患者的睡眠质量,减少患者疼痛感受,降低不良反应发生率,值得临床推广使用。

Objective:To observe the effect of acupuncture analgesia on the therapeutic effect and sleep quality of patients undergoing lower limb fracture surgery.Methods:A total of 200 patients with lower limb fractures admitted to the department of orthopedics,97th Hospital of the Joint Logistic Support Force of the Chinese People′s Liberation Army from February 2019 to February 2022 were selected as the research objects.According to the random number table method they were divided into the observation group and the control group,with 100 cases in each group.The control group was given ketorolac tromethamine for labor pain treatment,and the observation group was given acupuncture combined with labor pain treatment on the basis of the control group.The bed activity time and total hospital stay time were compared between the two groups.Visual Analogue Scale(VAS)was used to compare the postoperative pain of the two groups,and Pittsburgh Sleep Quality Index(PSQI)was used to compare the difference of sleep quality between the two groups.The incidence of postoperative complications was compared between the two groups.Results:After intervention,the total hospital stay and ambulation time of the observation group were shorter than those of the control group,the VAS score of the observation group was significantly lower than the control group at 1 day and 7 days after operation,the PSQI score of the observation group was significantly lower than that of the control group at 1 day and 7 days after operation,and the incidence of adverse reactions in the observation group was significantly lower than that of the control group.The differences between the two groups were statistically significant(P<0.05).Conclusion:Acupuncture and moxibustion labor pain can effectively improve the treatment effect of patients with lower limb fracture surgery,improve the quality of sleep,reduce the pain experience of patients,reduce the incidence of adverse reactions of patients,which is worthy of clinical application.
