观察Roy适应模式改善食管早癌内镜下黏膜下剥离术后患者睡眠障碍的效果

To Observe the Effect of Roy Adaptation Mode on Sleep Disorders in Patients with Early Esophageal Cancer after Endoscopic Submucosal Dissection

目的:探讨Roy适应模式改善食管早癌内镜下黏膜下剥离术(ESD)后患者睡眠障碍的效果。方法:选取2020年4月至2022年4月江苏省人民医院消化内科收治的进行食管早癌内镜下ESD术患者80例作为研究对象,按照随机数字表发分为观察组合对照组,每组40例。对照组予以常规护理干预,观察组在对照组基础上给予Roy适应模式进行干预。采用汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)比较2组患者干预前后焦虑、抑郁状态的改善情况,采用匹兹堡睡眠指数量表(PSQI)比较2组患者干预前后睡眠质量的变化,采用健康调查简表(SF-36)比较2组患者的生命质量。结果:干预后,观察组HAMA、HAMD评分均显著低于对照组,观察组SF-36评分显著高于对照组,2组比较差异均有统计学意义(P<0.05)。结论:Roy适应模式可以有效降低食管早癌ESD术后患者睡眠障碍的发生,提高患者睡眠质量,缓解抑郁、焦虑情况,临床价值值得肯定。

Objective:To investigate the effect of Roy adaptation model on sleep disorders in patients with early esophageal cancer after endoscopic submucosal dissection(ESD).Methods:A total of 80 patients undergoing endoscopic ESD for early esophageal cancer admitted to the Department of Gastroenterology of Jiangsu Provincial People′s Hospital from April 2020 to April 2022 were selected as the research objects and divided into observation group group and control group according to the random number table,with 40 cases in each group.The control group was given routine nursing intervention,and the observation group was given Roy adaptation mode on the basis of the control group.Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to compare the improvement of anxiety and depression state between 2 groups before and after intervention.Pittsburgh Sleep Index Scale(PSQI)was used to compare the changes in sleep quality between 2 groups before and after intervention.The Brief Health Survey Form(SF-36)was used to compare the quality of life between 2 groups.Results:After intervention,HAMA,HAMD and HAMD scores in the observation group were significantly lower than the control group,while SF-36 scores in the observation group were significantly higher than the control group,the difference between the two groups was statistically significant(P s<0.05).Conclusion:Roy adaptation model can effectively reduce the occurrence of sleep disorders in patients with early esophageal cancer after ESD surgery,improve the sleep quality of patients,relieve depression and anxiety,and its clinical value is worthy of recognition.
