2型糖尿病患者胰岛素抵抗水平与冠状动脉病变复杂程度的相关性分析 被引量:2

摘要 目的探讨2型糖尿病患者胰岛素抵抗水平与冠状动脉病变复杂程度的相关性。方法回顾性分析117例2型糖尿病合并冠状动脉粥样硬化性心脏病患者的临床资料,根据患者的冠状动脉病变复杂程度评分(Syntax评分)分为复杂组(Syntax评分≥22分)和非复杂组(Syntax评分<22分)。比较两组体质量指数(BMI)、年龄、性别,同时检测空腹血糖、餐后2 h血糖、空腹胰岛素、餐后2 h胰岛素、高密度脂蛋白-C(HDL-C)、低密度脂蛋白-C(LDL-C)、脂蛋白[Lp(a)]、尿常规、肌酐及糖化血红蛋白(HbA1c)等,计算胰岛素抵抗指数(HOMA-IR),分析各项指标与冠状动脉病变复杂程度的相关性。结果复杂组空腹胰岛素水平、餐后2 h胰岛素水平、外周血管疾病比例及HOMA-IR指数均高于非复杂组(均P<0.05)。Syntax评分与空腹及餐后两2h胰岛素水平有显著相关性,与HOMA-IR指数以及外周血管疾病的比例也高度相关(均P<0.05)。结论2型糖尿病患者的胰岛素抵抗程度可能与冠状动脉病变的复杂程度直接相关。
