IL-6及LBP与未足月胎膜早破组织学绒毛膜羊膜炎的关系探讨

Relationship between IL-6, LBP and histologic chorioamnionitis in preterm rupture of membrane

目的:探讨IL-6及LBP与未足月胎膜早破组织学绒毛膜羊膜炎的关系。方法:选取2021年1月—12月在湖南省妇幼保健院住院并分娩的未足月胎膜早破(PPROM)孕妇99例,根据其胎盘病检有无提示组织学绒毛膜羊膜炎(HCA),分为实验组56例(并发HCA),对照组43例(未并发HCA),比较两组分娩前IL-6及LBP水平。结果:①实验组及对照组分娩前IL-6分别为(22.3±17.3)、(11.5±1.7)pg/mL,实验组高于对照组,差异有统计学意义(P<0.05)。②实验组及对照组分娩前LBP分别为(3771.6±1431.0)、(3908.0±1664.4)ng/mL,差异无统计学意义(P>0.05)。③IL-6预测PPROM合并HCA的受试者工作特性曲线下面积为0.79,灵敏度为54%,特异度为93%。结论:IL-6对于未足月胎膜早破合并组织学绒毛膜羊膜炎的预测有较好的特异度。

Objective To investigate the relationship between lL-6,LBP and histologic chorioamnionitis in preterm rupture of membrane.Methods A total of 99 inpatients with Preterm premature rupture of membranes(PPROM)and delivery in Hunan Provincial Maternal and Child Health Care Hospital from January to December,2021,classified into HCA groups(56 cases)and non-HCA group(43 cases)according to placental pathology.Laboratory test results before delivery such as IL-6 and LBP of two groups were collected and the differences of the two research factors were compared among two groups.Results①The levels of IL-6 in HCA group and non-HCA group before delivery were(22.3±17.3)and(11.5±1.7)pg/mL,respectively,and HCA group was significantly higher than non-HCA group,and the difference between the two groups was statistically significant(P<0.05).).②The LBP of HCA group and non-HCA group before delivery were(3771.6±1431.0)and(3908.0±1664.4)ng/mL respectively,and there was no significant difference between the two groups(P>0.05).③The area under the receiver operating characteristic curve of IL-6 for predicting PPROM combined with HCA was 0.79,with a sensitivity of 54%and a specificity of 93%.Conclusion IL-6 has good specificity in predicting preterm premature rupture of membranes with histological chorioamnionitis.
