基于4C延续护理理念的出院准备服务模式的构建与实践

Construction and practice of discharge preparation service model based on the concept of 4C continuation nursing concept

目的 构建基于4C延续护理理念的出院准备服务模式并观察评价实践效果。方法 2021年6月医院成立院内出院准备服务小组,构建基于4C延续护理理念的出院准备服务模式。并于2021年6—12月将该模式在医院内外科进行实践验证,将2021年1—5月医院神经外科、心内科、产科及骨科治疗的100例患者作为对照组,将2021年6—12月上面4个科室的患者作为观察组,比较两组患者自我护理能力以及患者满意程度。结果 观察组患者的日平均更换尿管、更换胃管以及伤口换药次数明显少于对照组,差异有统计学意义(P<0.05);观察组住院患者出院后的自我护理能力评分均高于对照组,差异有统计学意义(P<0.05);观察组住院患者的住院满意程度以及自我效能水平均高于对照组,差异有统计学意义(P<0.05)。结论 将基于4C延续护理理念的出院准备服务模式应用在医院内、外科病房中,能够降低护士工作量,提高患者服务满意度及出院后自我护理能力。

Objective To construct the discharge preparation service model based on the 4C continuation nursing concept,to observe and evaluate the practice effect. Methods In June 2021, the hospital set up a hospital discharge preparation service team to build a discharge preparation service model based on the concept of 4C continuation care. From June to December 2021, this model was verifed in the internal and surgery department. 100 patients in neurosurgery, cardiology, obstetrics and orthopedics from January to May 2021 were treated as the control group, and patients from the above four departments from June to December 2021 were taken as the observation group to compare the self-care ability and patient satisfaction of the two groups. Results The average daily change of urinary catheter, gastric tube, and wound dressing change were signifcantly less than the control group, the difference was statistically signifcant(P<0.05);the self-care ability score of the observation group was higher than the control group, the difference was statistically signifcant(P<0.05);the inpatient satisfaction and self-efficacy of the observation group were higher than the control group, the difference was statistically significant(P<0.05).Conclusion The application of discharge preparation service model based on the 4C continuation nursing concept in hospitals and surgical wards could reduce the workload of nurses and improve patients’ service satisfaction and self-care ability after discharge.
