补肾健脾汤联合左卡尼汀治疗慢性肾衰竭临床研究

Clinical Study on Bushen Jianpi Tang Combined with L-Carnitine for Chronic Renal Failure

目的:观察补肾健脾汤联合左卡尼汀治疗慢性肾衰竭(CRF)的临床疗效。方法:将100例CRF患者随机分为观察组和对照组各50例,对照组采用静脉注射左卡尼汀注射剂治疗,观察组在对照组基础上联合补肾健脾汤辨证治疗。比较2组临床疗效及治疗前后的主要症状积分、肾功能指标、炎症因子水平,并记录治疗过程中不良反应发生情况。结果:观察组临床总有效率86.00%,高于对照组64.00%(P<0.05)。治疗前,2组主要症状积分比较,差异无统计学意义(P>0.05);治疗后,2组主要症状积分均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗前,2组血尿素氮(BUN)、血清肌酐(SCr)、肌酐清除率(CCr)、24 h尿蛋白定量水平比较,差异无统计学意义(P>0.05);治疗后,2组BUN、SCr、24 h尿蛋白定量水平均降低(P<0.05),CCr升高(P<0.05),且观察组BUN、SCr、24 h尿蛋白定量水平低于对照组(P<0.05),CCr高于对照组(P<0.05)。治疗前,2组白细胞介素-6 (IL-6)、肿瘤坏死因子-α (TNF-α)及超敏C-反应蛋白(hs-CRP)水平比较,差异无统计学意义(P>0.05);治疗后,2组IL-6、TNF-α、hs-CRP水平均降低(P<0.05),且观察组低于对照组(P<0.05)。2组不良反应比较,差异无统计学意义(P>0.05)。结论:采用补肾健脾汤辨证施治联合左卡尼汀治疗CRF患者可明显提高治疗效果,改善患者的主要中医症状、肾功能和微炎症状态,延缓疾病进展,且具有较好的安全性。

Objective:To observe the clinical effect of Bushen Jianpi Tang combined with L-carnitine for chronic renal failure(CRF).Methods:A total of 100 cases of CRF patients were randomly divided into the observation group and the control group,with 50 cases in each group.The control group was treated with L-carnitine,and the observation group was treated with L-carnitine combined with Bushen Jianpi Tang.The scores of main symptoms,the indexes of kidney function and the levels of inflammatory factors before and after treatment as well as the clinical effects were compared between the two groups;the incidence of adverse reactions during treatment was recorded.Results:The total clinical effective rate was 86.00% in the observation group,higher than that of 64.00% in the control group(P0.05).After treatment,the scores of main symptoms in the two groups were decreased(P0.05).After treatment,the levels of BUN,SCr and 24-hour urine protein excretion in the two groups were decreased(P0.05).After treatment,the levels of IL-6,TNF-α and hs-CRP in the two groups were decreased(P0.05).Conclusion:The therapy of Bushen Jianpi Tang combined with L-carnitine for CRF patients can significantly enhance the curative effect,improve the main Chinese medicine symptoms,kidney function and microinflammation status and delay the disease progression,with good safety.
