龙胆泻肝汤加减联合耳穴压豆辅助常规西药治疗不寐合并原发性高血压病临床研究

Clinical Study on Modified Longdan Xiegan Tang Combined with Auricular Point Seed-Pressing Assisting Routine Western Medicine for Insomnia Complicated with Essential Hypertension

目的:观察龙胆泻肝汤加减联合耳穴压豆辅助常规西药治疗不寐合并原发性高血压病的临床疗效。方法:选取不寐合并原发性高血压病患者62例,采用随机数字表法分为观察组和对照组各31例。2组均予以常规高血压管理,对照组予阿普唑仑睡前口服,观察组在对照组基础上予龙胆泻肝汤加减联合耳穴压豆治疗。比较2组临床疗效、24 h动态血压指标、中医证候积分、匹兹堡睡眠质量指数(PSQI)评分及生活质量评分,并记录2组不良反应发生情况。结果:观察组总有效率为93.55%,高于对照组74.19%,差异有统计学意义(P<0.05)。治疗前,2组24 h平均舒张压(24 h DBP)、24 h平均收缩压(24 h SBP)比较,差异无统计学意义(P>0.05)。治疗后,2组24 h DBP、24 h SBP较治疗前下降(P<0.05),观察组下降程度高于对照组(P<0.05)。治疗前,2组中医证候积分、PSQI评分及生活质量评分比较,差异无统计学意义(P>0.05)。治疗后,2组中医证候积分、PSQI评分及生活质量评分较治疗前下降(P<0.05),且观察组各项评分均低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:龙胆泻肝汤加减联合耳穴压豆辅助常规西药治疗不寐合并原发性高血压病的临床疗效较好,可改善患者的睡眠质量和临床症状,提高生活质量。

Objective:To observe the clinical effect of modified Longdan Xiegan Tang combined with auricular point seed-pressing assisting routine western medicine for insomnia complicated with essential hypertension.Methods:A total of 62 cases of patients with insomnia complicated with essential hypertension were selected and divided into the observation group and the control group according to the random number table method,with 31 cases in each group.Both groups were given routine hypertension management;the control group was given Alprazolam orally before sleep,and the observation group was additionally treated with modified Longdan Xiegan Tang combined with auricular point seed-pressing based on the treatment of the control group.The clinical effects,24-hour ambulatory blood pressure indexes,traditional Chinese medicine(TCM) syndrome scores,Pittsburgh Sleep Quality Index(PSQI) scores and scores of quality of life in the two groups were compared.The incidence of adverse reactions in the two groups were recorded.Results:The total effective rate was 93.55% in the observation group,higher than that of 74.19% in the control group,the difference being significant(P0.05).After treatment,the levels of 24-hour DBP and 24-hour SBP in the two groups were decreased when compared with those before treatment(P0.05).After treatment,the scores of TCM syndromes,PSQI and quality of life in the two groups were decreased when compared with those before treatment(P0.05).Conclusion:Modified Longdan Xiegan Tang combined with auricular point seed-pressing assisting routine western medicine has a good curative effect in treating insomnia complicated with essential hypertension,which can improve sleep quality,clinical symptoms and quality of life in patients.
