二甲双胍联合屈螺酮炔雌醇片(Ⅱ)治疗多囊卵巢综合征的临床效果及对激素、糖脂代谢指标、卵巢血流情况的影响

Clinical effect of metformin combined with drospirenone and ethinylestradiol tablets(Ⅱ)in the treatment of polycystic ovary syndrome and its influences on hormones,glucose and lipid metabolism indexes and ovarian blood flow

目的探讨二甲双胍联合屈螺酮炔雌醇片(Ⅱ)治疗多囊卵巢综合征(PCOS)的临床效果。方法选取我院2018年5月至2021年5月收治的80例PCOS患者作为研究对象,随机将其分为对照组和观察组,各40例。对照组给予屈螺酮炔雌醇片(Ⅱ)治疗,观察组给予屈螺酮炔雌醇片(Ⅱ)+二甲双胍治疗。比较两组的治疗效果、激素水平、糖脂代谢指标、子宫内膜厚度及卵巢血流情况。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,两组的雌二醇(E2)、促黄体生成素(LH)、催乳素(PRL)水平均降低,促卵泡生成素(FSH)水平均升高,且观察组优于对照组(P<0.05)。治疗后,两组的空腹血糖(FPG)、稳态模型评估-胰岛素抵抗指数(HOMA-IR)、甘油三酯(TG)、总胆固醇(TC)水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组的子宫内膜厚度均减小,搏动指数(PI)、收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)均升高,且观察组优于对照组(P<0.05)。结论二甲双胍联合屈螺酮炔雌醇片(Ⅱ)治疗PCOS的效果突出,可有效调节机体激素、糖脂代谢情况,改善患者的卵巢血流情况,值得临床推广。

Objective To investigate the clinical effect of metformin combined with drospirenone and ethinylestradiol tablets(Ⅱ)in the treatment of polycystic ovary syndrome(PCOS).Methods A total of 80 PCOS patients admitted in our hospital from May 2018 to May 2021 were selected as the research objects and randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with drospirenone and ethinylestradiol tablets(Ⅱ),and the observation group was treated with drospirenone and ethinylestradiol tablets(Ⅱ)+metformin.The therapeutic effect,hormones levels,glucose and lipid metabolism indexes,endometrial thickness and ovarian blood flow were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of estradiol(E2),luteinizing hormone(LH)and prolactin(PRL)in the two groups decreased,the levels of follicle stimulating hormone(FSH)increased,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of fasting plasma glucose(FPG),homestasis model assessment for insulin resistance(HOMA-IR),triglyceride(TG)and total cholesterol(TC)in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the endometrial thickness of the two groups decreased,the pulsatility index(PI),peak systolic velocity(PSV)and end diastolic velocity(EDV)increased,and those in the observation group were better than the control group(P<0.05).Conclusion Metformin combined with drospirenone and ethinylestradiol tablets(Ⅱ)in the treatment of PCOS has a prominent effect,it can effectively regulate the metabolism of hormones and glycolipids,and improve the ovarian blood flow of patients,which is worthy of clinical promotion.
