合理情绪治疗法联合米氮平对抑郁症患者的影响

Effect of Rational Emotional Therapy Combined with Mirtazapine in Depressed Patients

目的:探析合理情绪治疗法联合米氮平对抑郁症患者抑郁情绪、自杀风险、药物依从性及不良反应的影响。方法:选择重庆市第十一人民医院2021年1-12月就诊的90例抑郁症患者,采用随机数字表法分为两组,各45例。两组均给予米氮平治疗,对照组给予常规用药指导,观察组在对照组基础上联合合理情绪治疗法,连续治疗2个月。比较两组干预前后抑郁情绪[汉密顿抑郁量表(HAMD)、抑郁自评量表(SDS)]、自杀风险[自杀风险评估量表(NGASR)]、抗抑郁药物依从性[中文版抗抑郁药物依从性量表(ADCQ)]及不良反应发生率。结果:干预前,两组的HAMD、SDS、NGASR评分比较,差异无统计学意义(P>0.05);干预后,观察组的HAMD、SDS、NGASR评分低于对照组(P<0.05)。观察组ADCQ量表的4个维度(医患关系认知、自主性维持、对抗抑郁药物的积极信念、家人支持)及总分均高于对照组(P<0.05)。观察组的不良反应发生率为4.44%,低于对照组的22.22%(P<0.05)。结论:合理情绪治疗法联合米氮平应用于抑郁症患者中的临床效果显著,可有效改善患者抑郁情绪,提高抗抑郁药物依从性,降低自杀风险及不良反应发生率,值得临床推广。

Objective:To explore the effects of rational emotional therapy combined with Mirtazapine on depressive mood,suicide risk,drug compliance and adverse reactions in patients with depression.Method:A total of 90 patients with depression in Chongqing 11th People’s Hospital from January to December 2021 were selected and divided into two groups according to the method of random number table method,with 45 cases in each group.Both groups were treated with Mirtazapine,the control group was given routine medication guidance,and the observation group was treated with rational emotional therapy on the basis of the control group.The depression[Hamilton depression scale(HAMD),self rating depression scale(SDS)],suicide risk[Nurses’Global Assessment of Suicide Riske(NGASR)],antidepressant drug compliance[Chinese version of antidepressants compliance questionnaire(ADCQ)]before and after the intervention and the incidence of adverse reactions were compared between the two groups.Result:Before intervention,there were no significant differences in HAMD,SDS and NGASR scores between the two groups(P>0.05).After intervention,the scores of HAMD,SDS and NGASR in the observation group were lower than those in the control group(P<0.05).The four dimensions(cognition of doctorpatient relationship,maintenance of autonomy,positive belief of antidepressant drugs,family support)and total scores of the ADCQ scale in the observation group were higher than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was 4.44%,which was lower than 22.22%in the control group(P<0.05).Conclusion:The clinical effect of rational emotional therapy combined with Mirtazapine in patients with depression is significant,which can effectively improve the depressive mood of patients,improve the compliance of antidepressants,reduce the risk of suicide and the incidence of adverse reactions,and is worthy of clinical promotion.
