摘要
目的 分析血清谷丙转氨酶(ALT)水平与高尿酸血症的关联。方法 研究对象来自2021年10月参加入学体检的11 858名大学新生,收集人口学特征和身高、体质、血压等体格检查数据,检测空腹血清ALT、尿酸、肌酐等水平。根据血清ALT水平,采用logistic回归模型及广义相加模型分析其与高尿酸血症的关联。结果 共纳入11858名大学生,平均年龄为(21.9±4.5)岁,其中男生5 907名(49.8%),高血压者314人(2.7%)。高尿酸血症检出3 372例(28.4%),尿酸水平及高尿酸血症检出率随血清ALT水平上升而升高。多因素logistic回归模型结果显示,不论是按ALT水平四分位数分组还是按连续性变量分组,ALT水平都与高尿酸血症患病风险成正相关,OR(95%CI)值分别为3.24(2.80~3.76)(Q4 vs Q1)和2.12(1.97~2.29)。广义相加模型分析发现该关联呈线性关系。结论 高血清ALT水平与高尿酸血症的患病风险增加存在独立关联。
Objective To analyze the relationship between serum alanine aminotransferase(ALT) level and hyperuricemia.Methods The subjects were 11 858 freshmen who participated in the physical examination in October 2021. The demographic characteristics, height, weight, blood pressure, and other physical examination data were collected, and the levels of fasting serum ALT, uric acid, creatinine, etc. were detected. The logistic regression model and generalized additive model were used to analyze the relationship between serum ALT levels and hyperuricemia.Results A total of 11 858college students were enrolled, with an average age of(21.9 ± 4.5) years. Among them, 5 907 were male(49.8%) and 314 were hypertensive(2.7%). Hyperuricemia was detected in 3 372 cases(28.4%), and the uric acid level and hyperuricemia detection rate increased with the increase in serum ALT level. The results of the multivariate logistic regression model showed that ALT level was positively correlated with the risk of hyperuricemia, whether grouped by quartile of ALT level or by a continuous variable. OR(95% CI) values were 3.24(2.80-3.76)(Q4 vs Q1) and 2.12(1.97-2.29), respectively. The generalized additive model analysis found that the correlation was linear.Conclusion High serum ALT level is independently associated with an increased risk of hyperuricemia.
