阿替普酶静脉溶栓联合双联抗血小板治疗超早期脑梗死的效果分析

Effect of intravenous thrombolysis with arteprase combined with dual antiplatelet therapy on ultra-early cerebral infarction

目的分析阿替普酶(rt-PA)静脉溶栓联合双联抗血小板治疗超早期脑梗死的临床效果。方法抽取2018年12月至2021年12月漯河市中心医院收治的超早期脑梗死患者116例,按照治疗方案将其分为对照组和观察组,每组58例。对照组给予常规治疗及双联抗血小板(阿司匹林联合氯呲格雷)治疗,观察组在对照组治疗基础上加用rt-PA静脉溶栓治疗,记录并比较治疗前后两组神经功能评分及日常生活能力评分。结果治疗后,两组美国国立卫生研究院卒中量表(NIHSS)、改良Rankin评分量表(mRS)评分较治疗前下降,且观察组NIHSS、mRS评分下降幅度大于对照组,差异有统计学意义(P<0.05)。治疗后,两组BI评分较治疗前增加,且观察组BI评分增加幅度大于对照组,差异有统计学意义(P<0.05)。结论超早期脑梗死患者采用rt-PA静脉溶栓联合双联抗血小板治疗效果理想,有利于改善神经功能,提高患者生活能力。

Objective To analyze the clinical effect of intravenous thrombolysis with arteprase(rt-PA)combined with dual antiplatelet therapy on ultra-early cerebral infarction.Methods A total of 116 patients with ultra-early cerebral infarction admitted to Luohe Central Hospital from December 2018 to December 2021 were selected,and they were divided into control group and observation group according to treatment plans,with 58 cases in each group.The control group was treated with routine treatment and dual antiplatelet therapy(aspirin combined with clopidogrel),and the observation group was given rt-PA intravenous thrombolysis based on the treatment of the control group.The neurological function scores and daily living ability of the two groups were recorded and compared before and after treatment.Results After treatment,the National Institutes of Health Stroke Scale(NIHSS)score and modified Rankin rating scale(mRS)score of the two groups decreased,compared with those before treatment,and decrease ranges of the observation group were greater than those of the control group(P<0.05).After treatment,the Barthel index(BI)of the two groups were higher than those before treatment,and the increase of BI of the observation group was greater than that of the control group(P<0.05).Conclusions In patients with ultra-early cerebral infarction,rt-PA intravenous thrombolysis combined with dual antiplatelet therapy has an ideal effect,which is conducive to improving neurological function and patients’living ability.
