失眠患者选择中医护理技术的整体情境分析

Overall situation of insomnia patients choosing TCM nursing techniques

目的探讨失眠患者选择中医护理技术的相关情境因素,为推广中医护理适宜技术提供参考。方法利用扎根理论的质性研究方法,采用目的抽样法,于2021年10—11月选取在同济大学附属东方医院中医护理门诊就诊的15例失眠患者为研究对象进行半结构式访谈,采用三级编码的方法对访谈资料进行整理和分析。结果本研究显示失眠患者选择中医护理技术的整体情境可分为主体情境(失眠严重程度的感知、中医护理的认知和信念、中医护理技术的效果体验)、主体条件(基础条件、心理准备、知识素养)、外部因素(中医护理适宜技术的推广、医院-社区-家庭一体化建设),共包含18个概念。3个情境组成要素相互区别又共同作用,激发、调节、推动着失眠患者的中医护理行为。结论医护人员应区分主体情境、主体条件和外部因素的不同作用,不断调整、改进中医护理措施,推广中医护理失眠的适宜技术,满足患者的护理需求。

Objective To explore the situational factors of insomnia patients'choice of traditional Chinese medicine(TCM)nursing technology,so as to provide reference for the promotion of appropriate TCM nursing technology.Methods Using the qualitative research method of grounded theory and the purposive sampling method,15 insomniacs who visited the TCM Nursing Clinic of Shanghai East Hospital affiliated to Tongji University from October to November 2021 were selected for semi-structured interview.The interview data were collated and analyzed using the three-tier coding.Results The overall situation for insomnia patients to choose TCM nursing technology could be divided into subject situation(the perception of insomnia severity,the cognition and belief of TCM nursing,and the effect experience of TCM nursing technology),subject conditions(basic conditions,psychological preparation,knowledge literacy),and external factors(the promotion of appropriate TCM nursing technology,and the construction of hospital community family integration),including 18 concepts.The three situational elements were different from each other and worked together to stimulate,regulate and promote the TCM nursing behavior of insomnia patients.Conclusions The medical and nursing staff should distinguish the different roles of the subjective situation,subjective conditions and external factors,constantly adjust and improve the TCM nursing measures,promote the appropriate TCM nursing technology in insomnia,and meet the nursing needs of patients.
