眶颧颌骨折手术时机与预后的探讨

Exploration of relationship between surgical timing and prognosis of orbito-zygomatico-maxillary fractures

摘要 目的探讨眶颧颌骨折手术时机的选择与预后的关系。方法回顾性分析自2011年1月至2021年1月,中国医科大学附属第一医院眼科收治的185例眶颧颌骨折患者按照受伤到接受手术时间分为3组:早期组(病程≤2周)37例;中期组(>2周~4周)92例;晚期组(>4周~8周)56例。总结并比较各组患者手术耗时及术后治愈率。结果手术耗时:早期组(82.30±14.31)min,中期组(90.22±12.54)min,晚期组(121.10±24.17)min,三组间两两比较,差异均有统计学意义(P<0.05)。早期组治愈率为89.2%(33/37),中期组治愈率为83.7%(77/92),晚期组治愈率为67.9%(38/56)。早期与中期组间比较,差异无统计学意义(P>0.05);早期与晚期组、中期与晚期组之间比较,差异均有统计学意义(P<0.05)。结论眶颧颌骨折患者伤后手术时机以4周内为宜,2周内可能会更好。 Objective To explore the relationship between surgical timing and prognosis of orbito-zygomatico-maxillary fractures.Methods From January 2011 to January 2021,185 patients with orbito-zygomatico-maxillary fractures were divided into three groups according to the duration of disease:37 cases in the early group(disease duration≤2 weeks),92 cases in the intermediate group(>2 weeks to 4 weeks)and 56 cases in the late group(>4 weeks to 8 weeks).The operation time and postoperative recovery rate were compared and summarized.Results The operation time was(82.30±14.31)min in the early group,(90.22±12.54)min in the intermediate group and(121.10±24.17)min in the late group.There were statistically significant differences among three groups.The recovery rate was89.2%(33/37)in the early group,83.7%(77/92)in the intermediate group and 67.9%(38/56)in the late group.There was no statistically significant difference between the early group and the intermediate group(P>0.05).There was statistically significant difference between the early group and the late group,and between the intermediate group and the late group(P<0.05).Conclusion Patients with orbito-zygomatico-maxillary fractures are repaired within 4 weeks after injury had appropriate outcomes,and within 2 weeks may be better.
