社会歧视对我国流动人口健康状况的影响

Impact of Social Discrimination on Health Status of Floating Population in China

目的:探讨基本公共卫生服务利用情况在流动人口遭受的社会歧视与其健康状况之间的中介效应,为改善流动人口健康状况提供参考。方法:利用2017年全国流动人口动态监测调查数据,采用logit回归法分析流动人口所受社会歧视、健康状况及基本公共卫生服务利用情况之间的关系,并用KHB效应分解法检验基本公共卫生服务利用情况的中介作用。结果:流动人口所受的社会歧视影响其健康状况,社会歧视对健康状况的直接效应为-0.010,其95%CI为(-0.030,-0.019);基本公共卫生服务利用情况在社会歧视与健康状况之间起中介作用,中介效应占总效应的30.7%。结论:流动人口所受社会歧视能直接影响其健康状况,又可以通过影响基本公共卫生服务利用情况间接影响健康状况。因此应在减少流动人口在流入地受歧视机会的同时,完善基本公共卫生服务利用情况,提高人群健康水平。

Objective:To explore the mediating effect of basic public health service utilization between social discrimination that floating population received and their health status,so as to provide references for improving the health status of the floating population.Methods:The logit regression method was used to analyze the relationship of social discrimination that floating population received,health and utilization of basic public health services among floating population by using data from the 2017 national floating population dynamic monitoring survey,and the mediating effect of utilization of basic public health services was examined by using the KHB effect decomposition method.Results:The social discrimination that floating population received affected health status,the direct effect of social discrimination on health status was-0.010,and 95%CI was(-0.030,-0.019).The basic public health service utilization played a mediating role between social discrimination and health status,and the mediating effect accounted for 30.7%of the total effect.Conclusion:Social discrimination that floating population received can affect health status directly,and it can also affect health status by affecting the utilization of basic public health services indirectly.Therefore,we should improve the utilization of basic public health services and promote the health of the population,while reducing the opportunities of floating population to receive social discrimination in the place they flow in.
