认知症居家患者高质量照护的内在逻辑与路径选择

The Internal Logic and Path Selection of High-quality Home Care for People with Dementia

认知症居家患者高质量照护是当前医养结合领域亟待突破的重要瓶颈。高质量照护需求的满足能有效提升认知症患者生活质量,降低家庭照护者压力与照护负担。当前我国针对认知症患者的居家照护服务处于探索阶段,高质量照护是目前认知症照护的目标导向。本文基于对国内外现有认知症居家照护模式的总结,提出了家庭视域下认知症居家高质量照护逻辑模型,明确了家庭为高质量照护基石、照护需求为牵引、专业照护力量介入为补充的内在逻辑,并尝试从稳固家庭照护持续性、激活并满足高质量照护需求、联动专业照护力量等方面提出高质量照护等路径选择,以期为促进认知症高质量、全面性、持续性照护服务供给提供参考。

High-quality care for people with dementia is an important bottleneck to be broken in the field of the integration of medical treatment and care.The meet of high-quality care demands can effectively improve the quality of life of people with dementia and reduce the pressure and care burden of family caregivers.At present,the home care services for people with dementia are in the exploratory stage in China,and the high-quality care for families with dementia is an urgent goal to be realized.Based on the summary of current home care models for dementia at home and abroad,the present article proposes a logical model of high-quality home care for dementia from the perspective of family,and clarifies the internal logic that family is the cornerstone of high-quality care,care demand is the traction,and the intervention of professional care force is the supplement.This article also tries to put forward the path selection of high-quality care from the aspects of stabilizing the continuity of family care,activating and meeting the demands for high-quality care,and linking professional care forces.The purpose of this study is to provide reference for promoting the supply of high-quality,comprehensive and continuous care services for the people with dementia.
